Your task is to apply the theoretical ideas and findings of how groups work to help understand aspects of group/team development and functioning in the following social care case study/scenario.

You are working as a team leader in a residential service. There are a number of new staff that have just started within the team and a number of staff who have worked together for up to 10 years. There are two team leaders, eight social care workers and six young people between the ages of 12 – 17 with mild intellectual disabilities. The young people’s families are very involved in the service and the team are regularly in contact with family members. All of the staff get monthly supervision provided by you and the team leaders. While supervision is provided for you and the team leaders regularly by senior management. You run weekly team meetings although these are not running as effectively recently as you would link them to. There appears to be subgroups developing within the staff team and you are aware that there have been some disagreements between staff.

The quest to understand the lived experience and the philosophical structures of experience and consciousness is the focus of a movement that began in the early years of the 20th Century. The movement is called phenomenology because the movement explores the lived experience of phenomena. Edmund Husserl and Martin Heidegger were two of the prominent philosophers who developed this method of inquiry though each took a significantly different philosophical approach to understanding human experience. In this assignment, you will discuss the psychology research methods that emerged from the philosophies of Husserl and Heidegger and the researchers who developed those methods.

Purpose: 

The purpose of this project is to apply leadership concepts to a crisis leadership situation and to make recommendations for a crisis-ready culture.

The key to doing well in this assignment is to use course materials from all seven weeks. While there is a focus on Week 7 content, students are expected to apply the concepts learned from weeks 1 through 6 as well. Moreover, students are expected to apply examples from the case scenario to concepts learned from the course materials. A Snapshot of the Case and the Underlying Issues:

This case focuses on a crisis that was created because of poor leadership decisions and behaviors. The decisions made by Higgins created the problem, but at the core of this problem was the poor leadership of Brown. Higgins made a decision to purchase genetically modified Echinacea for short-term gain. She made this decision without consulting with anyone. But the crisis could have been prevented by Brown. Brown demonstrated:

  • the wrong leadership style in giving Higgins free rein
  • low emotional intelligence by not demonstrating control of his emotions and not managing conflict well
  • that he was not an Authentic leader, in that he didn’t clearly communicate the values most important to him and to Biotech (the core values). Knowing that the core values would have possibly allowed Higgins to reframe her decision making (in other words she would have understood sustainability and customer-centricity come before short term profits). Moreover, Brown did not take ownership of the problem – another sign of an unauthentic leader.

Finally, this case demonstrates what can happen when culture, structure, and strategy do not align. After working through these leadership issues, students will provide recommendations to make Biotech crisis-ready.

Discussion: The Balance Sheet Dilemma

An industrial firm is confronted with the dilemma of whether it should include—on its balance sheet—a very costly liability for the clean-up of a hazardous waste pool that has accumulated over the past several years as a direct by-product of its operations. The company agrees that the cost of cleaning up the hazardous waste pool is a cost of doing business.

However, including this massive financial liability in the company’s financial statements now will have the effect of immediately decreasing the total value of the company. In turn, because the company’s worth will be deflated, the company’s current stockholders will be harmed financially because the company will have lost much of its current value.

In contrast, if the company does not include the liability on its balance sheet, the company will clearly be overvalued, and will likely be viewed as dishonest in disclosure—and anyone buying stock (or ownership) in the company will also be unknowingly buying shares of a company that is overvalued (i.e., the balance sheet will be misleading, showing the company as having greater net value than it actually has).

Let’s assume that there are no accounting rules nor is there any clear guidance as to what this company should do. No matter what the company decides, innocent persons—existing shareholders (if disclosure is made) or future shareholders (if disclosure is not made and is later required) will be seriously harmed as a result of either decision.

Case 17-1 When Parents Refuse to Give Up1

Nine-year-old Yusef Camp began experiencing symptoms soon after eating a pickle bought from a street vendor. He felt dizzy and fell down, he could not use his legs, and he began to scream. By 10:00 p.m., he was hallucinating and was transported to the DC General Hospital by ambulance. He went into convulsions. His stomach was pumped, and they found traces of marijuana and possibly PCP. He soon stopped breathing, and by the next morning, brain scans showed no activity.

Four months later, Yusef’s condition had not changed. The physicians believed his brain was not functioning and wanted to pronounce him dead based on brain criteria. Several difficulties were encountered, however. First, there was some disagreement among the medical personnel over whether his brain function had ceased completely. Second, at that time the District of Columbia had no law authorizing death pronouncement based on brain criteria. It was not clear that physicians could use death as grounds for stopping treatment. Most important, Ronald Camp, the boy’s father, protested vigorously any sug- gestion that treatment be stopped. A devout Muslim, he said, “I could walk up and say unplug him; but for the rest of my life I would be thinking, was I too hasty? Could he have recovered if I had given it another 6 months or a year? I’m leaving it in Almighty God’s hand to let it take whatever flow it will.”

The nurses involved in Yusef’s care faced several problems. Maggots were found growing in Yusef’s lungs and nasal passages. His right foot and ankle became gangre- nous. He showed no response to noises or painful stimuli. The nurses had the responsi- bility not only for maintaining the respiratory tract and the gangrenous limb, but also for providing the intensive nursing care needed to maintain Yusef in debilitated condition on life support systems. Had the aggressive care been serving any purpose, they would have been willing to provide it no matter how repulsive the boy’s condition was and in spite of there being many other patients desperately needing their attention. However, some of the nurses caring for Yusef were convinced that they were doing no good what- soever for the boy. They believed they were only consuming enormous amounts of time and hospital resources in what appeared to be a futile effort. In the process, other patients were not getting as much care as would certainly be of benefit to them. Could the nurses or the physicians argue that care should be stopped because he was dead? Could they overrule the parents’ judgment about the usefulness of the treatment even if he were not dead? Could they legitimately take into account the welfare of the other patients and the enormous costs involved when deciding whether to limit their atten- tion to Yusef?

Promoting Learning-Centered Lessons in Hybrid Environments

Hybrid courses allow students to have the best of both worlds. Students who prefer to have face-to-face interaction with their instructor and peers but also have the option to study at home should consider hybrid courses. Hybrid courses offer flexibility in programming and tends to demonstrate the best of both face-to-face education and online innovation (Bristol & Zerwekh, 2011). Hybrid courses require both synchronous and asynchronous interaction. Asynchronous interactions do not depend on a specific time or place. This can be done through email, discussion forum, podcasts, and archive video and audio streams (Billings & Halstead, 2016). Synchronous interaction occurs in real time. Examples of this may include live video conferencing, chat rooms, or webcasts (Billings & Halstead, 2016). Some of the advantages of a hybrid environment are improved course organization, effective record keeping, more communication, multimodal learning experiences, and multilayered assessment activities (Bristol & Zerwekh, 2011). There are various methods that nurse educators can use in the hybrid environment to enhance the instruction of their intended learners

Bradshaw & Hultquist (2017) explains how a blended learning environment utilizing a time saving method known as “flipping”. Students review learning material outside of class before a session. Doing so allows classroom time to used for active learning and discussion rather than listening to a lecture (Bradshaw & Hultquist, 2017). Having access to technology and knowledgeable of its use is essential in flipped classrooms (Billings & Halstead, 2016).

In one study, nurse educators reported use of e-learning (Moodle) in courses using quizzes, notes/PowerPoints, videos, networking, and chats/forums/blogs (D’Souza, Karkada, & Castro, 2014). Moodle enhances communication in teaching, exchange of information with students, and accessibility to Moodle facilitates learning (D’Souza, et. al., 2014). The students had an opportunity to interact with instructors and peers and they felt a “connection” in the learning process (D’Souza, et. al., 2014).

     As is the case with many of the Online sections for this course, I have gotten some good questions related to the Assignment 3 content and I wanted to give you some clarifying guidance on what is expected.  This assignment is arguably the most technically challenging, especially when you do not have the opportunity for a class session to talk through the content.

     For the assignment, you are to use the PDF document attached to that specific content item (in Module 4) in order to help you complete questions 1 thru 5.  The PDF shows you two records from two separate tables housed in two separate databases (MPD and CSS).  The chart at the bottom of the content area is there only for reference to help you understand what a combined data warehouse model may look like as you brought those two database tables together.  You are not expected to fill out this chart or re-create it in any fashion.

-For question 1, you need to identify the primary key of one of the tables on the MPD database.  Note (as shown in the question) that the way parent objects and children are reflected in database terms is to list the PARENT.CHILD separated by a period.  

-For question 2, you are being asked to try and create a basic SQL query to extract certain information from one of the SALES tables (the one from the CSS database).  SQL queries have three parts (refer to sample figure 5.10 as a guide) and here is some additional information on each: 

– a SELECT clause, which is where you state what fields you want to see in your results.  Fields are properly written as TABLE.Field1, TABLE.Field2, etc.

– a FROM clause, which is where you state from what table you want to pull the results.  The example in the text has two tables, but you are only pulling from one (SALES) for this assignment.

– a WHERE clause, which is where you state how you want to filter the results.  This is where you define the specific constraints.  Without this, it will pull all fields you asked for in the SELECT, from the table you asked for in the FROM line.

An example would be: SELECT TABLE.Field1, TABLE.Field2, TABLE, Field3

Note that in the actual query, there is no mention of the database you are pulling from (MPD or CSS in this example).  Rather, this is done at a level above any queries since a SQL query only goes against a single database.  Thus, you do not need to have any reference to the MPD or CSS database as part of your query.

– For questions 3 & 4, you should refer to section 5.4 where there are topics on information assurance and quality of information to help you answer this.  Specifically for #3, you should do some analysis on the two tables from the attachment and identify some inconsistencies that may cause issues should you simply merge the two tables together into a single warehouse.

– For question 5, refer to section 10.2 where there are actually six business intelligence ‘capabilities’ identified (HINT:  first one listed in ‘Production Reports’).   You should select five of them and provide your response for this question.

Career Counseling as an Integral Part of Human Development

Career development is not a one-time event, rather it is a process that occurs across the lifespan and is an integral part of human development.  Further, the counseling that may be offered to support career development must also be offered from a lifespan perspective, with counselors supporting a client’s unique needs at the various stages of their life and career.  Zunker (2016) also points out that changing cultural and environmental systems can effect human development, and best practice involves case conceptualization from a holistic perspective.  Humans are actively growing and changing throughout their lives, and their vocational interests, goals, and preferences are no exception.  Career counseling is a dynamic and lifelong process that evolves with each client throughout the course of their life.   

The Developmental Lifespan Model Influence on Career and Educational Planning, Placement, and Evaluation

From a mental health counseling perspective, the developmental lifespan model of career planning is highly influential.  Mental health counselors will need to be prepared to address all phases of career counseling in all phases or stages of a client’s life.  Career and education planning begin in early childhood and continue throughout the course of life.  Mental health counselors will need to be aware of the foundational career needs of the children they serve, and be prepared to focus on improved social skills, industry, and communication skills.  When working with adolescents, mental health counselors also need to be aware of the importance of developing quality relationships outside of their family, and how these interpersonal skills will benefit them later in the workforce.  Also, mental health counselors will need to understand life stages when selecting assessment tools, conducting evaluations, and placing individuals in jobs.   

The Influence of Career Counseling When Working With Children, Adolescents, and Older Adults

Career counseling can easily be integrated into work with children by focusing on the foundational skills necessary for successful education, vocational, and social experiences.  Some of the foundational skills that counselors can focus on with children include prosocial skills, positive work habits, diversity skills, pleasing personality traits, and entrepreneurship (Gysbers, 2013).  Counseling work with adolescents can begin to focus on planning, goal setting, and decision making skills, along with a focus on curricula that supports a possible career direction.  Interestingly, Newman and Newman (2012) highlight the concept of career maturity, which suggests postponing career decisions until an adolescent or young adult matures and gains valuable life experience.  Career counseling with adults in the new workforce places more emphasis on career development than remaining at a particular company (Zunker, 2016).  Retirement counseling should also be highlighted as individuals move to transition from the world of work to increased volunteer and leisure opportunities.  Finally, more retired individuals are going back to work on a part time basis in order to supplement their retirement income, and may require counseling to make this change. 

Appropriate Career Counseling Models for Children and Adolescents, Including Erik Erickson’s Model

Stage theorists such as Erik Erickson conceptualize career counseling from the developmental life stage that a particular client is navigating.  Between the ages of 6 to 11, for example, children are actively learning a variety of social, academic, and work related skills that will create a foundation for later more complex career development.  This stage of development is also associated with achievement of self-efficacy and an understanding of the importance of productivity.  Adolescents are actively working on the developmental task of achieving a group and individual identity and avoiding isolation.  Adolescents work hard to expand their social circles and distance themselves from their parents in an effort to achieve independence (Newman & Newman, 2012).  An overarching principal associated with Erickson’s stage model is that children or adolescents who fail to successfully achieve their developmental tasks may require special supports later in life (Zunker, 2016).  Career counseling from this stage model would consider this developmental information as the foundation from which to create a comprehensive career plan. 

Other career counseling models appropriate for children and adolescents include Super’s self-concept theory, Krumboltz’s learning theory, and cognitive development theory.  Because elementary school students are busy forming their identity or self-concept through their childhood relationships, Super’s self-concept theory may be applied (Zunker, 2016).  Krumboltz’s learning theory looks at the way that children and adolescents utilize observation in learning new things, and are able to adapt their behavior based on this observational learning (Zunker, 2016).  Piaget’s cognitive development theory is also a stage theory in that it views children’s knowledge acquisition as developing in specific steps or levels through their environmental engagement.

Newman & Newman (2012) stated that career identities are “a well-integrated part of [people’s] personal identities rather than as activities from which they are alienated or by which they are dominated” (p. 412). Career counseling across the lifespan has implications in all fields of counseling practices. As it pertains to mental health counseling, career counseling becomes an integrated conversation about the wants, needs, & desires a person has to have a satisfied existence.

Developmental Lifespan from a MH Perspective

Zunker (2016) stated that early life experiences tend to influence later life decisions. As it applies to career counseling, this is the core and foundation for how young children begin to view the world and all it has to offer. For example, children who have parental figures who exhibit hard work ethic are likely to influence their young children especially if it is reinforced with at-home activities (e.g., chores). These experiences, along with other life experiences, are likely to shape what a child decides to do. From a MH perspective, because the child’s feelings about these practices greatly challenge or confirm their beliefs, it will affect their behaviors.

Erikson’s Psychosocial Developmental Model illustrates the challenges that people face at different stages of their life development. What made Erikson’s model much more appealing (versus Freud’s Psychosexual Theory) is that it included polarities that challenged each individual’s relationship to his/her culture, family, and life environment (Syed and McLean, 2015). So as it is applied to career counseling in the cases of young children and adolescents, Erikson’s developmental model provides a theoretical explanation for the decisions and choices one makes at certain times of his/her life.

Branje, Lieshout, & Gerris (2007) studied personality development across adolescence and adulthood to see if the Big Five personality factors (extraversion, agreeableness, conscientiousness, emotional stability, and openness to experience) changed as individuals aged and gained experience. Their research suggested that males have fewer changes than females, but both sexes showed increasing signs of maturity and adaptation as they aged. The importance of their study was that it showed that personality continued to develop during the middle adulthood potentially because of the delegation of new responsibilities (i.e., parenthood). As it is applied to career counseling, the changing course of one’s career can have profound impact on their livelihood and decisions that (in)directly affect how one views his/her future.

Thoughts from the “Other Side”

Based upon Maslow’s (1943) Hierarchy of Needs, there exist several, innate physiological needs during infancy: food, water, shelter, sleep, air (breathing), excretion, and sensory satisfaction (e.g., touch, taste, hear, feel, and smell; Daniels, 1992; Maslow, 1943; and Seeley, 1992). The early, formative years allow the child to experience the world through their parent’s permission. Fast forward to adulthood, and these same basic needs are still required; however, they have evolved into something much more complex. When integrated with the work life, it is not surprising that these same needs are still needed in the workplace environment; however, they are ascribed new titles or new entitlements. For example, infants have the need for food; employees have the need for a clean environment to enjoy said food. Another example: infants have a need for sleep; employees need an hour per day for a break to do with it whatever they would like. These needs have never left; they have just evolved. The career demands that one has available to him/her that are most desirable are the attributes the (s)he will seek. In these instances, it will be necessary to determine if a client is okay with where (s)he is at this junction of his/her life. If they are not, it will be important to determine how career counseling and lifespan developmental theory can be influential in assisting with producing changes.

Assessing And Treating Clients With Dementia

BACKGROUND

Mr. Akkad is a 76 year old Iranian male who is brought to your office by his eldest son for “strange behavior.” Mr. Akkad was seen by his family physician who ruled out any organic basis for Mr. Akkad’s behavior. All laboratory and diagnostic imaging tests (including CT-scan of the head) were normal.

According to his son, he has been demonstrating some strange thoughts and behaviors for the past two years, but things seem to be getting worse. Per the client’s son, the family noticed that Mr. Akkad’s personality began to change a few years ago. He began to lose interest in religious activities with the family and became more “critical” of everyone. They also noticed that things he used to take seriously had become a source of “amusement” and “ridicule.”

Over the course of the past two years, the family has noticed that Mr. Akkad has been forgetting things. His son also reports that sometimes he has difficult “finding the right words” in a conversation and then will shift to an entirely different line of conversation.

SUBJECTIVE

During the clinical interview, Mr. Akkad is pleasant, cooperative and seems to enjoy speaking with you. You notice some confabulation during various aspects of memory testing, so the PMHNP performs a Mini-Mental State Exam. Mr. Akkad scores 18 out of 30 with primary deficits in orientation, registration, attention & calculation, and recall. The score suggests moderate dementia.

MENTAL STATUS EXAM

Mr. Akkad is 76 year old Iranian male who is cooperative with today’s clinical interview. His eye contact is poor. Speech is clear, coherent, but tangential at times. He makes no unusual motor movements and demonstrates no tic. Self-reported mood is euthymic. Affect however is restricted. He denies visual or auditory hallucinations. No delusional or paranoid thought processes noted. He is alert and oriented to person, partially oriented to place, but is disoriented to time and event [he reports that he thought he was coming to lunch but “wound up here”- referring to your office, at which point he begins to laugh]. Insight and judgment are impaired. Impulse control is also impaired as evidenced by Mr. Akkad’s standing up during the clinical interview and walking towards the door. When the PMHNP asked where he was going, he stated that he did not know. Mr. Akkad denies suicidal or homicidal ideation.

Diagnosis: Major neurocognitive disorder due to Alzheimer’s disease (presumptive)

Guidance to Student

At this point, the client is reporting no side effects and is participating in an important part of family life (religious services). This could speak to the fact that the medication may have improved some symptoms. The PMHNP needs to counsel the client’s son on the trajectory of presumptive Alzheimer’s disease in that it is irreversible, and while cholinesterase inhibitors can stabilize symptoms, this process can take months. Also, these medications are incapable of reversing the degenerative process. Some improvements in problematic behaviors (such as disinhibition) may be seen, but not in all clients.

At this point, the PMHNP could maintain the current dose until the next visit in 4 weeks, or the PMHNP could increase it to 6 mg orally BID and see how the client is doing in 4 more weeks. Augmentation with Namenda is another possibility, but the PMHNP should maximize the dose of the cholinesterase inhibitor before adding augmenting agents. However, some experts argue that combination therapy should be used from the onset of treatment.

Finally, it is important to note that changes in the MMSE should be evaluated over the course of months, not weeks. The absence of change in the MMSE after 4 weeks of treatment should not be a source of concern.
