Case Study: Fetal Abnormality

Jessica is a 30-year-old immigrant from Mexico City. She and her husband Marco have been in the United States for the last three years and have finally earned enough money to move out of their Aunt Maria’s home and into an apartment of their own. They are both hard workers. Jessica works 50 hours a week at a local restaurant and Marco has been contracting side jobs in construction. Six months before their move to an apartment, Jessica finds out she is pregnant.

Four months later, Jessica and Marco arrive at the county hospital, a large, public, nonteaching hospital. A preliminary ultrasound indicates a possible abnormality with the fetus. Further scans are conducted, and it is determined that the fetus has a rare condition in which it has not developed any arms and will not likely develop them. There is also a 25% chance that the fetus may have Down syndrome.

Dr. Wilson, the primary attending physician, is seeing Jessica for the first time, since she and Marco did not receive earlier prenatal care over concerns about finances. Marco insists that Dr. Wilson refrain from telling Jessica the scan results, assuring him that he will tell his wife himself when she is emotionally ready for the news. While Marco and Dr. Wilson are talking in another room, Aunt Maria walks into the room with a distressed look on her face. She can tell that something is wrong and inquires of Dr. Wilson. After hearing of the diagnosis, she walks out of the room wailing loudly and praying aloud.

Marco and Dr. Wilson continue their discussion, and Dr. Wilson insists that he has an obligation to Jessica as his patient and that she has a right to know the diagnosis of the fetus. He furthermore is intent on discussing all relevant factors and options regarding the next step, including abortion. Marco insists on taking some time to think of how to break the news to Jessica, but Dr. Wilson, frustrated with the direction of the conversation, informs the husband that such a choice is not his to make. Dr. Wilson proceeds back across the hall, where he walks in on Aunt Maria awkwardly praying with Jessica and phoning the priest. At that point, Dr. Wilson gently but briefly informs Jessica of the diagnosis and lays out the option for abortion as a responsible medical alternative, given the quality of life such a child would have. Jessica looks at him and struggles to hold back her tears.

Jessica is torn between her hopes of a better socioeconomic position and increased independence, along with her conviction that all life is sacred. Marco will support Jessica in whatever decision she makes but is finding it difficult not to view the pregnancy and the prospects of a disabled child as a burden and a barrier to their economic security and plans. Dr. Wilson lays out all of the options but clearly makes his view known that abortion is “scientifically” and medically a wise choice in this situation. Aunt Maria pleads with Jessica to follow through with the pregnancy and allow what “God intends” to take place and urges Jessica to think of her responsibility as a mother.

History of Presenting Case

The family consisted of 40 -year-old John M, and his wife Mary M., who is 32 years old, and they have no children. The clients were seen in a mental crisis center. Initially, Mr. John had been voluntarily admitted to the unit due passive suicidal thoughts, depression, and alcohol dependence. Mr. John had been recently fired from his job due to showing at work drunk. Couple therapy was recommended to speed up his recovery and maintain sobriety.

In the session, Mr. John self-reported mood as depressed, and he acknowledged his drinking problem disrupting his life, and he was prepared to fully participate in the recovery for the sake of his well-being and his marriage. On the hand, his wife Mary, appeared anxious, and frustrated. She blamed the husband for all the misery of the family. She was much worried about their future as the husband had lost his job. However, she was willing to support him in recovery.

Psychiatric History: Mr. John has battling depression for six years, and his PCP prescribed antidepressant (Zoloft) last year but he abruptly stopped taking them as he felt that the medication was affecting his sexuality. He did not follow up with his PCP. He self-medicates with alcohol. His wife reported history of anxiety and occasional panic attacks, she was taking fluoxetine 40mg daily. The medication was prescribed by PCP.

Psychiatric Hospitalization: It was the first time for Mr. John to hospitalized in mental facility.

His wife reported no history of psychiatric hospitalization.

Medical History: They both denied any medical history

Psychosocial History: They both said they smoked marijuana occasionally. The wife works as cashier in a local gas station. They live in a one- bedroom apartment.

Differential Diagnosis:

Major Depression Disorder F 32

Mr. John presented with depressed mood and reported history of loss of interest in pleasurable activities, sleep disturbance and suicidal thoughts lasting more than 2 weeks. In addition, to meet the diagnostic criteria the symptoms had caused clinically significant distress or impairment in social, occupational, or other important areas of functioning( American Psychiatric Association, 2013)

Alcohol Use disorder AUD

To be diagnosed with an AUD, individuals must meet certain diagnostic criteria. Some of these criteria include problems controlling intake of alcohol, continued use of alcohol despite problems resulting from drinking, development of a tolerance ( Grant& Saha, 2015).

Generalized Anxiety Disorder: Ms. Mary exhibited some of the signs of generalized anxiety. Diagnostic criteria symptoms include excessive anxiety and worry as well as restlessness, fatigue, irritability, tension, and sleep problems (Watson& Greenberg, 2017)

Ethical and Ethical Implications of Counselling the Couple

  Family members often experience resentment about past substance abuse and fear and distrust about the possible return of substance abuse in the future (O’ Farrell& Fals-Stewart, 2013)In the case of John and Mary, there is a higher probability that there will be uncertainty and distrust during the recovery. It may be unethical to tell directly his wife that her anxiety and blame towards her husband may demoralize him and impede the recovery. However, it may be beneficial as feelings experienced by the substance abuser and the family often lead to an atmosphere of tension and unhappiness in couple and family relationships( O’Farrell & Fals-Stewart, 2013).

The Behavior Learning Theory is the best approach to improve nutrition and healthy habits among children and families. With the behavioral approach, I will teach families how to incorporate healthy habits into their daily lifestyle. To accomplish this, I will create a pamphlet for families to take home with them that will include the information covered in my presentation. The pamphlet will also include a list of local resources that the family can use to continue their journey. During my presentation, I will teach families about the important food groups and how much of each should be including in each meal. My presentation will focus on healthier alternatives and ways to modify their current eating habits rather than restricting them from foods they love. I will have healthy plate samples and I will allow each family to create a healthy plate that includes all food groups as well as the proper proportions. We will also discuss grocery shopping habits and how to avoid certain isles when shopping with children.

Resistance is the major barrier I plan to have to deal with. I will engage in regular communication with each family and identify any resistance barriers early on. I will also personally visit any families that are missing regularly scheduled follow-ups. I also think financial constraints might be an issue so I will help families find healthy options within their budget.

They will not be allowed to just watch, but they will become part of the presentation. They will be part of demonstrations and role-playing activities. Furthermore, I will ask them for personal accounts of where they learned their nutrition habits and how they can improve them. I will maintain eye contact for all participants, and I will monitor body language for signs of confusion or understanding. I will conclude by presentation by obtaining contact information from all participants for future follow-up.

Watson’s 10 Caritas Processes

The word “Caritas” is Latin. It means cherish, appreciate, and give special or loving attention with charity, compassion, and generosity of spirit. Caritas is very fine and precious, and must be actively cultivated to be sustained (Watson, 2008).

You must be able to explain the financial implications associated with the measure, as well as its importance in terms of policy, regulation, and finance. This is another good opportunity to consult with a key stakeholder within the organization. Talking to someone who works closely with the organization’s finances can help you develop a deeper understanding of the financial aspects of compliance.
