Written Assignment: Issues and Dilemmas of Contemporary Public Administration
The concept of public interest appears, at least from what has been presented to me in the required reading for this class, to be a highly questionable concept. There are those that would beg the question does public interest even exist? If that answer is in the affirmative, how does one gauge public interest? Can it even be accurately gauged? Personally I agree with the people that state the concept is highly
incoherent. It would seem to me that the concept of public interest is highly fluid and ever-changing, just as society is ever-changing. Certainly administrators and legislators can act in the best interest of the public, but for how long are their actions relevant, this has been a widely neglected question throughout my readings. Everyone will agree that society is ever-changing, and therefore the needs of society are in turn also always changing, but at the same time the majority of theorists and scientists on the subject go on and on about history, and tradition, how things have been done and often times how things are still being done that way. I would never be foolish enough to say that learning from ones past is a bad idea, studying history is vital, but how can so many people be studying the history of politics and administration and the evolution of public interest and yet have nothing new to show for it. Have we not learned from the mistakes of the past, from the inequalities and inefficiencies of the past? From a historical standpoint our democracy is at the precipice of its life, from a purely historical perspective our democracy has lived a long full life, and will inevitably fail just as others have. That proposition saddens me and I sincerely hope that we start learning from our mistakes, and right now for our survival, our mistakes are all we really need to concentrate on, we are the albatross, we will either evolve to suit the needs of our ever growing technological population, or we will die and if the United States of America fails at democracy when once we were a shining light I shudder to imagine what would happen to the rest of the world. Until we have better means by which to create an accurate consensus on public interest, so as to provide the highest standard of life for the citizens, public interest will still be a point of confusion and ignorance among all levels of government.
All of this reading about responsibility gives me flashbacks of being lectured by my parents for not taking out the trash and expecting an allowance, it is absolutely mind boggling that responsibility within the government has been so grievously overlooked, and not to mention more than a little frightening that measures for holding not only public administrators, but any member of government, responsible for their
actions, or inactions, have been so absent. Yes, I understand that some measures are in place to punish those that have so blatantly broken the law and gotten caught, but for the most part it seems that the only real punishment for those people who are elected, is not being re-elected. And for those appointed the only real punishment in place for irresponsible actions is internal pressure to resign. It is no wonder there is so much written on the topic of administrative responsibility, the current system has no real mechanisms for enforcement for codes of ethics, we have apparently been running the government on the honor system assuming that all those who fight for administrative positions are going to be good, honest, hard working individuals with the wellbeing of the public in mind. That was a really bad assumption to make, human nature itself dictates that we each tend ourselves first, when in history did politicians and administrators become an exception to that rule, they are still human after all – right? What ethical obligations that a public administrator must face, are entirely individual based, it depends almost solely on the values of the person, and with whom their allegiance lies. For instance an unmarried person, with no dependants, may have little moral imperative and therefore no ethical responsibility in their mind, to families. They may direct the solutions to problems that arise in their field in such a way that families are not a primary concern. It would be easy to assume that because someone has made it into a position of public administration that they feel morally obligated to do everything in their power to serve the public, this would create an ethical code for them that steers them to serve the public at large. They may also feel ethically obligated through that to not play favorites any certain interest groups that may try to distract his attention to their more selfish needs and away from the needs of the general public. In that same token the administrator would also be morally and ethically obligated to not ignore a minority in the community, the administrator would treat everyone that they must deal with equally and with ample enthusiasm, as doing something ethically sound often times is a great motivator.
I am of the opinion that privatizing the government would be a wonderful thing, we have allowed, as a nation, the government to grow to massive proportions while at the same time do nothing but complain about how inefficient it is. Despite all its inefficiencies we still allow it to grow, why? That answer is simple and Colin Wilson said it best “The average man is conformist, accepting miseries and disasters with the stoicism of a cow standing in the rain”. That is what we are now; we’re cows standing in the rain. We have grown inherently lazy, with this sense of entitlement that has been nurtured by the government itself, as the government seeks to always expand its power. The government does not want intelligent, motivated, out spoken citizens, it does not want citizens that would seek to challenge its authority, and it knows that human nature dictates seeking out the path of least resistance. So the government will go ahead and provide unemployment for an eternity, food, housing, healthcare – I shudder to type that one – and the people will eat it up because in their mind it is free, and that just goes to show the ignorance of people these days, it does not take a rocket scientist to figure out that nothing is free, someone has to pay for it, and it is more becoming the minority that pays for it. By minority I mean the successful
entrepreneurs, the people creating jobs, and also in this turbulent economy the people lucky enough to still have jobs. I also mean the wealthy, all of these people paying taxes, and if they really thought about it they would be furious to know that their hard earned money, taken by the government, was being used
to PAY people NOT to work, to feed people that intentionally have six kids so welfare will pay out more just because they are too lazy to try and find a job. Freedom was never free, ever, and people need to remember that, the freedom that we enjoy was worked hard for, it was fought for, and we have to bite and claw and scratch and dig to keep every ounce of power that the government does not really need, out of their hands so that we can keep that freedom. As essential as government is, it can just as easily enslave as set free. What do I think should be privatized? I don’t suppose just saying everything is enough so I
will elaborate a little.
To start with education, public schools in many areas of the country failed long ago, the moment they started lowering the passing grades so that failing children could pass those schools failed. It has also been a common myth that private schools are better than public schools, and it is true. If you were to privatize education, sure a lot of teachers would lose their jobs, and they will probably deserve it, it will
also take a huge funding burden off of the government, pass those savings onto the tax payers and all of a sudden private school isn’t a real cost, the children still get better education, better prepared for life as an adult, sounds to me like a classic win-win scenario. Sure you can ask how all the people not paying taxes will see this increase in funds to be able to afford school and that is simple, if they are not paying taxes they do not deserve rewards for it. If they are here illegally and not paying taxes why should we
pay to educate their children? If they are mooching off welfare out of pure laziness why would we pay to educate their gaggle of children? Considering school is probably the best babysitter those people have I would almost venture to bet if we did that those people would have to get a job, then they’d be paying taxes, and would have the money for school.
Next I would privatize the post office, considering all of the other shipping options out there, a private
company would step in to fill the gap left behind by the post office, and it would very likely be more efficient and better managed, and there goes yet another funding burden from the government, amazing how that works, turn things over to the private sector not only do they stop costing the government money, but they actually increase tax revenue so they start making the government money. An interesting concept, making the government money, we keep on this train of thought and they might actually be able to balance the budget, wouldn’t that be something?
I would also privatize police departments; it has been long well known that private security companies do a better job enforcing the peace where they are employed to enforce the peace than the current government tormented police departments. This is of course no fault of the policemen themselves, they do everything that they can with the tools that they are given, and for what they have they do an exceptional job, but imagine what the current police departments could manage if their budget were doubled, or tripled, without any governmental meddling. What would pay for these privatized police? That is simple too; they would still be paid via taxes, through government contracts. The company itself would still govern how they ran their business, the government contracts out to private security companies all the time, and run like a business the efficiency of the new privatized police would be significantly higher than the current police that are heavily restricted by arbitrary government policy and meddling.

Case 4–1 NATIONAL STANDARDS ON CULTURALLY AND LINGUISTICALLY APPROPRIATE SERVICES (CLAS)
In 1997, the Office of Minority Health (OMH) in the U.S. Department of Health and Human Services began work on national standards for culturally and linguistically competent health care. The stated goal was to help reduce health disparities. OMH published draft standards in December 1999 and solicited public comment through a variety of channels over a 4-month period. On December 22, 2000, it published the final standards. Although the standards are primarily directed at health care organizations, OMH encourages their use by individual providers as well as by policy makers, accreditation and credentialing agencies, purchasers, patients, advocates, educators, and the health care community in general (OMH, 2001).
CULTURALLY COMPETENT CARE (GUIDELINES FOR ACTIVITIES RECOMMENDED BY OFFICE OF MINORITY HEALTH FOR ADOPTION AS MANDATES BY FEDERAL, STATE, AND NATIONAL ACCREDITING AGENCIES)

Health care organizations (HCOs) should ensure that patients/consumers receive effective, understandable, and respectful care from all staff members that is provided in a manner compatible with their cultural health beliefs and practices and preferred language.

ORGANIZATIONAL SUPPORTS FOR CULTURAL COMPETENCE
Standards 8–13 are guidelines for activities recommended by the Office of Minority Health for adoption as mandated by federal, state, and national accrediting agencies. Standard 14 is suggested for voluntary adoption by HCOs.

HCOs should develop, implement, and promote a written strategic plan that outlines clear goals, policies, operational plans, and management accountability/oversight mechanisms to provide culturally and linguistically appropriate services.

HCOs should conduct initial and ongoing organizational self-assessments of CLAS-related activities and are encouraged to integrate cultural and linguistic competence-related measures into their internal audits, performance improvement programs, patient satisfaction assessments, and outcomes-based evaluations.

HCOs should ensure that data on individual patient’s/consumer’s race, ethnicity, and spoken and written language are collected in health records, integrated into the organization’s management information systems, and periodically updated.
Standard 11
HCOs should maintain a current demographic, cultural, and epidemiological profile of the community as well as a needs assessment to accurately plan for and implement services that respond to the cultural and linguistic characteristics of the service area.

Case 6–1 GLOBAL MEDICAL COVERAGE
BACKGROUND
Blue Ridge Paper Products, Inc. (BRPP) in Canton, NC is a paper company making predominantly food and beverage packaging. It was the largest employer left in Western North Carolina in 2006, with 1,300 covered employees in the state and 800 elsewhere. Started as a Champion Paper plant in 1908, it was purchased by the employees and their union (a United Steelworkers local) in May 1999 with the assistance of a venture capital firm and operates with an Employee Stock Ownership Plan (ESOP). To purchase it, the employees agreed to a 15% wage cut and frozen wages and benefits for seven years. From the buyout through the end of 2005, the company lost $92 million and paid out $107 million in health care claims. It became profitable in 2006. Maintaining health benefits for members and retirees is a very high priority item with the employees and the union, although retiree medical benefits have been eliminated for salaried employees hired after March 1, 2005.
BRPP employees are “predominantly male, over 48, with decades of services and several health risk factors. They work 12-hour, rotating shifts, making it extremely difficult to manage health conditions or improve lifestyle” (Blackley, 2006). The ESOP has worked hard to reduce its self-insured health care costs. Health insurance claims for 2006 had been estimated at $36 million, but appeared likely to hold near $24 million, which is still 75% above the 2000 experience. A volunteer Benefits Task Force of union and nonunion employees worked to redesign a complex benefit system. After two years of 18% health care cost increases, the rate of growth dropped to 2% in 2003. It was 5% in 2004 and a negative 3% in 2005.
Programs initiated in 2001 included a plan offering free diabetic medications and supplies in return for compliance and a tobacco cessation plan with cash rewards. In 2004, the company opened a full-service pharmacy and medical center with a pharmacist, internist, and nurses. In 2005, it began a Population Health Management program. Covered employees and spouses who completed a health risk assessment were rewarded with $100 and assigned a “personal nurse coach.” The nurse coach assists those who are ready to change to set individual health goals and choose from among one or more of 14 available health programs, which may include “cash rewards, waived or reduced co-pays on over 100 medications, free self-help medical aids/equipment, educational materials, etc.”
Where BRPP could not seem to make headway was with the prices paid to local providers. Community physicians refused deeper discounts. Even banding together in a buying cooperative with other companies could not move the local tertiary hospital to match discounts offered to regionally dominant insurers. This hospital was not distressed and had above-average operating margins.
Articles on “medical tourism” in the press and on television attracted the attention of benefits management. Reports were of high quality care at 80% or less of U.S. prices with good outcomes. BRPP contacted a company offering services at hospitals in India, IndUShealth in Raleigh, NC, and began working on a plan to make its services available to BRPP employees.
IndUShealth
IndUShealth provides a complete package to its U.S. and Canadian clients, including access to Indian superspecialty hospitals that are Joint Commission International accredited and to specialists and supporting physicians with U.S. or U.K. board certification. It arranges for postoperative care in India and for travel, lodging, and meals for the patient and an accompanying family member—all for a single package price. For example, it represents the Wockhardt hospitals in India, which are Joint Commission International accredited and affiliated with Harvard Medical International. Other Indian hospitals boast affiliations with the Johns Hopkins Medical Center and the Cleveland Clinics.
Mitral Valve Replacement
One of the first cases considered was a mitral valve replacement. IndUShealth and BRPP sought package quotes from a number of domestic medical centers and could get only one estimate. That quote, from the University of Iowa academic medical center, was in the $68,000 to $98,000 range. The quote from India was for $18,000 including travel, food, and lodging for the patient and one companion. Testifying before the U.S. Senate Special Committee on Aging, Mr. Rajesh Rao, IndUShealth CEO, (2006) cited the following costs.

EMPLOYEE PARTICIPATION
To encourage employee participation, BRPP prepared a DVD on its medical tourism initiative, which it called Global Health Coverage. It outlined the opportunities and described the Indian facilities and credentials. The next step was to be a trip by an employee “due diligence” committee to India to inspect facilities and talk with doctors. Then they would discuss how to handle the option in the next set of union negotiations.
SENATE HEARINGS
On June 27, 2006, the U.S. Senate Special Committee on Aging held hearings entitled “The Globalization of Health Care: Can Medical Tourism Reduce Health Care Costs?” Both BRPP and IndUShealth presented together with others.
When testifying to the Senate subcommittee, Bonnie Grissom Blackley, benefits director for BRPP, concluded:
Should I need a surgical procedure, provide me and my spouse with an all expense-paid trip to a Joint Commission International-approved hospital, that compares to a 5-star hotel, a surgeon educated and credentialed in the U.S., no hospital staph infections, a registered nurse around the clock, no one pushing me out of the hospital after 2 or 3 days, a several-day recovery period at a beach resort, email access, cell phone, great food, touring, etc., etc. for 25% of the savings up to $10,000 and I won’t be able to get out my passport fast enough.
BLUE RIDGE PAPER PRODUCT’S TEST CASE
The test case under the new arrangement was a volunteer, Carl Garrett, a 60-year-old BRPP paper-making technician who needed a gall bladder removal and a shoulder repair. He reportedly was looking forward to the trip in September 2006, accompanied by his fiancée. A 40-year employee approaching retirement, he would be the first company-sponsored U.S. worker to receive health care in India. The two operations would have cost $100,000 in the United States but only $20,000 in India. The arrangement was that the company would pay for the entire thing, waive the 20% co-payment, give Garrett about a $10,000 incentive, and still save $50,000.
The United Steel Workers Union national office objected strongly to the whole idea, however, and threatened to file for an injunction. The local district representative commented, “We made it clear that if healthcare was going to be resolved, it would be resolved by modifying the system in the U.S., not by offshoring or exporting our own people.” USW President Leo Gerard said, “No U.S. citizen should be exposed to the risk involved in travel internationally for health care services” and sent a letter to members of Congress that included the following (Parks 2006):
Our members, along with thousands of unrepresented workers, are now being confronted with proposals to literally export themselves to have certain “expensive” medical procedures provided in India.
With companies now proposing to send their own American employees abroad for less expensive health care services, there can be no doubt that the U.S. health care system is in immediate need of massive reform
The right to safe, secure, and dependable health care in one’s own country should not be surrendered for any reason, certainly not to fatten the profit margins of corporate investors.
The union also cited the lack of comparable malpractice coverage in other countries. The company agreed to find a domestic source of care for Mr. Garrett, but may continue the experiment with its salaried, nonunion employees. Carl Garrett responded unhappily, “The company dropped the ball …. people have given me so much encouragement,” he said, “so much positive response, and they’re devastated. A lot of people were waiting for me to report back on how it went and perhaps go themselves. This leaves them in limbo too” (Jonsson, 2006, p. 2).

Leadership- My first ranking of the ten principles for a police chief would be leadership. I feel this is important because if you cannot lead people or in this case a department and community, the program will not work. Leadership has many titles or styles and it does not matter which one the police chief is but he needs to be able to lead a team to a common goal. Some of the traits that a good leader should have are; honesty, focus, passion, respect and excellent persuasion abilities just to name a few (Farrell, 2011). Someone must lead the charge for change and implementation and a good leader as a police chief is the answer.
Educator, Motivator and Trainer- This principle comes in a close second as you must be able to take the vision of what the police department will look like, how it will interact with the community and who will be responsible for the different parts of the program. Implementing community policing is a tough job and you will need a leader who can motivate the parties to keep working towards the common goal. The chief will also have to train and educate those that need it so that they understand the process completely. If you can teach them, keep them motivated through the process, then this will allow for success at a much higher rate.
Commitment- This is pretty much what it says, you need a chief that is committed. I have seen so many leaders in our community come in, lay out a great plan and then after a few months you read where they are looking for another job or they have began another program and the original plan has not been completed. Commitment to the community and to the department is paramount for community policing to be successful.
Role Model- Having a chief that is a role model is a key for the solidifying of the process of getting the community to trust him or her and the department. People naturally will base their opinion off of the latest news or drama and so if the police chief has been involved in some questionable circumstances in disciplining his officers or other items, they will loose faith in the chief and ultimately the department. Trust from the community members is important in community policing and this allows for the program to grow and get implemented with little resistance and increased participation.
I do not think that the principles change over time but they will rise to more visibility depending on the situation. Once the community policing is in place and is operating, if a situation such as an office who has acted inappropriately occurs, it will be important for the chief to handle the situation properly and without issue in the eyes of the community. So as time goes on, they will rise and fall depending on the situation but they do not change as far as importance.
