For nurses who have entered the workforce within the last 10 years, paper charting may be something only talked about by older nurses, or utilized begrudgingly during downtime. Since the passage of the Health Information Technology for Economic and Clinical Health Act of 2009 (HITECH) that promoted the adoption and meaningful use of electronic health records (EHR), EHRs have become synonymous with medical care (McGonigle & Mastrian, 2015). While it may seem that EHRs are efficient and easy to use, the process of adopting these systems is not without challenges.

Resistance to Implementation

Staff resistance to the adoption of an EHR is one of the most challenging aspects of implementing the system in health care facilities. I have even heard grumblings from fellow nurses about finding another job when my facility spoke about initiating the change to EHRs. In fact, “leadership, planning, management, choices, and execution usually are more difficult than dealing with hardware and software choices” (Mooney & Boyle, 2011, p. S5)). It is so important to have buy-in from nursing staff, as they make up a significant portion of health care workforce. In order to combat this resistance, staff nurses should be involved in the transition process from the beginning. If nurses feel that they have stake in the process, they will feel more invested in the outcome and successful on-boarding of the new technology. I suggest having staff nurses participate in early planning meetings, and to train staff nurses to become ‘super users’ within the facility. It will be helpful during implementation for nurses to be assisted by peers versus outside professionals.

Flexibility and Expandability

When choosing an EHR to implement in your workplace, it is so important to plan for the future so that you can ensure that as your facility grows and changes, the EHR can flex with it. For example, in my winter job at a ski resort, electronic charting was implemented only last year. There was a lot of thought that went into the selection process, however, decision-makers did not plan for coordination with Ski Patrol’s electronic charting system. Unfortunately, DocuTap (used by our clinic) does not interoperate with any of the Ski Patrol software currently available. This poses a challenge, as much of the information needed at each place is similar and shared between disciplines. As it stands now, Ski Patrol continues to use paper charting, while the clinic uses DocuTap. We have to copy the information obtained by ski patrol and hand-deliver it to the clinic. To combat this issue, it is imperative that the development team evaluate all software options, as well as the tech support each one offers as this may be a higher start-up cost, but save money in the long-run (Mooney & Boyle, 2011).

Process Improvement

Once an EHR is implemented and the extra support staff is gone, the challenges do not end. Once nurses begin to use the technology more comfortably, they may identify a process that needs to be changed to make it more user-friendly or to better suit the needs of the staff. In order for staff to continue to use the technology appropriately, they need to feel that their input is valued and that there is the possibility for change and improvement within the software after it is implemented. My suggestion to help with this issue is to have a nurse liaison who has clinical experience within the organization and has an interest in nursing informatics. This individual’s role is to act as a leader by relaying staff concerns to the software company to see if there are changes that can be made to make the software more user-friendly or to more appropriately meet the needs of staff. “What health care needs most are effective leaders who understand innovation and how it spreads, who respect the diversity of change itself, and who can nurture innovation in all its rich and many costumes” (Hyrkas & Harvey, 2010, p. 1). The hospital I work for has a liaison employed by Cerner, the company who manages the software we use. This nurse was a staff nurse in the emergency department, then a clinical nurse educator. This system works very well for us.

Summary

In conclusion, there are several potential challenges to implementing EHRs, including: resistance to implementation, flexibility and expandability, and process improvement. Since the HITECH Act of 2009, EHRs have been initiated in every major medical facility in the country. Though not without obstacles, meaningful use of EHRs contribute to the safe, efficient, and accessibility of medical information.

The tympanic membranes are slightly reddened on the periphery, but otherwise normal in appearance. The throat is erythematous with 4+ tonsils and diffuse exudates. Anterior cervical nodes are readily palpable and clearly tender to touch on the left side. The child indicates that her throat hurts “a lot” and it is painful to swallow. Vital signs reveal a temperature of 102.8oF, a pulse of 128 beats per minute, and a respiratory rate of 24 beats per minute.

Scenario 2:

Jack is a 27-year-old male who presents with redness and irritation of his hands. He reports that he has never had a problem like this before, but about 2 weeks ago he noticed that both his hands seemed to be really red and flaky. He denies any discomfort, stating that sometimes they feel “a little bit hot,” but otherwise they feel fine. He does not understand why they are so red. His wife told him that he might have an allergy and he should get some steroid cream. Jack has no known allergies and no significant medical history except for recurrent ear infections as a child. He denies any traumatic injury or known exposure to irritants. He is a maintenance engineer in a newspaper building and admits that he often works with abrasive solvents and chemicals. Normally he wears protective gloves, but lately they seem to be in short supply so sometimes he does not use them. He has exposed his hands to some of these cleaning fluids, but says that it never hurt and he always washed his hands when he was finished.

Scenario 3:

Martha is a 65-year-old woman who recently retired from her job as an administrative assistant at a local hospital. Her medical history is significant for hypertension, which has been controlled for years with hydrochlorothiazide. She reports that lately she is having a lot of trouble sleeping, she occasionally feels like she has a “racing heartbeat,” and she is losing her appetite. She emphasizes that she is not hungry like she used to be. The only significant change that has occurred lately in her life is that her 87-year-old mother moved into her home a few years ago. Mom had always been healthy, but she fell down a flight of stairs and broke her hip. Her recovery was a difficult one, as she has lost a lot of mobility and independence and needs to rely on her daughter for assistance with activities of daily living. Martha says it is not the retirement she dreamed about, but she is an only child and is happy to care for her mother. Ashford General Hospital is a 263-bed regional hospital located in California, serving its community for more than 50 years. 1.Desertification is the persistent degradation of dryland ecosystems by climate changes and human activities. In 2000, drylands were home to one-third of the human population. Desertification can occur naturally due to droughts; however, humans play a huge role in the process as well. For example, diverting rivers from their natural path so that we can use the water for agriculture. The area would no longer have a water supply, and vegetation could potentially die off.

Unsustainable use of resources and the lack of adequate infrastructures can lead to land degradation. Furthermore, the removal of most vegetation is a major factor that causes degradation. We can help control this by integrating land and water management to protect soils from erosion, and other forms of degradation. We can also give local communities the capacity to prevent this process and to manage dryland resources more effectively.

 In the 1930’s, poor land use led to awful dust storm in the Great Plains. A combination of farmers over using the land’s resources and horrible droughts, led to one of the worst environmental disasters of the twentieth century. Fortunately, I do not live in an area that is affected by desertification. I live in a rural area on about twenty acres and have a small garden. I sometimes use pesticides to keep the bugs away. It is very hot and muggy here and bugs are an issue. The use of pesticides could be causing desertification by putting chemicals in the ground.

  Merriam-Webster’s dictionary describes desertification as the process by which an area becomes a desert.  It is the process where arable or fertile land become wastelands, drier and unproductive as they once were. 

  One of the ways that most everyone is indirectly responsible for desertification is through our use of wood.  Wood comes from places that are taking part in deforestation.  This has led to the extinction of some animals and the desertification of some lands.  Humans rely on wood for many reasons, from building homes, to fires, to using charcoal for cooking.  All of these things are taking part in the desertification of some lands.

  The Colorado Plateau is an area that is currently fighting desertification.  This is not so much desertification due to human impacts, more than the growth of plants and trees that are taking out other grasses and vegetation that certain wildlife needs for survival.  In order to combat this, vegetation is planted in certain places, and when there is intruder plants that invade, some controlled fires are used to take the area to a beginning phase in what it should be.

  Some of the major causes are deforestation that large logging companies partake in.  The only way to combat this is to ensure that wood is gathered from sources not using mass deforesting techniques, or planting trees in the same area that are forested.  Another major cause is over farming land, one way to combat this is by crop rotation where farmers rotate their crops to keep the land able to be farmed into the future and ensure that the minerals needed for farming are not just used up by farming.  Another cause would be overgrazing.  This is combated by the use of fences to ensure that different areas are grazed at different times of the year allowing for those areas that were grazed to grow again.

  In the 1930s, the major cause of the dust bowl were a huge drought, as well as over farming.  Farmers had yet to learn the art of crop rotations.  They had used up the nutrients of the soil so that nothing would grow.  Mix with this the drought that was seen and when the wind picked up, it blew dust around since there was no ground cover due to the over farming.  This ended when the rains came back ending the drought.

Desertification is a process in which an area becomes drier and drier until eventually it is transformed into a desert or a barren wasteland in comparison to what it once was. Desertification is caused by a variety of processes, one of those being the loss of vegetation needed to keep topsoil in place. A few other causes of desertification are overgrazing by livestock or improper rotation of crops. Farmers also have to be careful not only when planning what crops to plant, but also in how to water them. An overabundance of salt can occur severely reducing the soil’s ability to remain stable enough to promote plant growth. As you can see, human beings are a main contributor of desertification.  Humans are also known for using an area for all of its resources and once those resources are exhausted we move again and again, continuing a trail of desertification. 

     I am not currently located in an area of desertification. There are numerous small ponds, field, and livestock around the area I am in, but the grounds are fertile and promote crop growth. I have indirectly contributed to desertification by renting houses that were built after clear cutting an area of its trees. I may have directly contributed to desertification by driving my vehicle and increasing greenhouse gases in our atmosphere, which may contribute to global warming. Thus over time the increased temperatures could promote the process of desertification happening even quicker.

     The Great Plains “dust bowl” covered an area of 150,000 miles in the lands of Texas and Oklahoma. The combination of light soil, high winds, little rain, overgrazing of cattle, and human interaction all contributed to the dust bowl happening. Farmers took special note of the rising wheat prices and in their attempts to cash in on the big harvests began to plow over grasslands as well as knock down soil holding trees. Due to the unlivable conditions of the dust bowl, 2.5 million people fled the area. NASA has also concluded that cooler Pacific Ocean temperatures and warmer Atlantic Ocean temperatures created near perfect conditions for drought.

Prior to the September 11, 2001 terrorist attacks, the United States had a long record of safeguarding against home front attacks. When the 9/11 terrorist attacks occurred, many people, including key government officials, were shocked that a terrorist attack, especially one of such magnitude, could occur on American soil. Since then, homeland security has become a top priority. Structures, policies, and strategies in existence prior to 9/11 have been critically examined to determine inadequacies, and new structures, policies, and strategies have been developed. This course is designed to give you an understanding of terrorism, terrorist groups, and pre- and post-9/11 U.S. counterterrorism structures, policies, and strategies. Understanding these areas is fundamental to combating terrorism and predicting and preventing future terrorist attacks.

Terrorism by definition is the use of force or violence against persons or property in violation of criminal laws for purpose of intimidation, coercion, or ransom. Terrorist acts range from threats and assassinations to hijackings and bombings (FEMA, 2009). These acts are of extremism born from no single race, religion, national, or political affiliation. Terrorism is not a new phenomenon. It has existed in every era of history and continues to evolve. Likewise, terrorist groups such as Al Qaeda have evolved, adapted, and sometimes flourished despite heightened international efforts to combat terrorism and dismantle terrorist groups. The likelihood of terrorism ceasing to exist in the future is slim to none; however, there are strategies that can slow the promulgation of terrorist groups, stifle terrorist activities, and predict and prevent future terrorist attacks.

You begin your study of terrorism by examining its potential causes as well as the motivations of terrorists. In doing so, you might be surprised to learn that there is no one single cause of terrorism. Rather, there is a multitude of causes or contributing factors that may lead to terrorism. These causes or contributing factors include poverty, perceived social and political injustice, religious extremism, fanaticism, ethnic conflict, and inequality. Just as there is no one single cause of terrorism, there is no agreed-upon profile that applies to all terrorists. While many people may assume that terrorists generally are uneducated, mentally unstable, or have a history of criminal activity, this simply is not the case. Terrorists come from a wide array of backgrounds and in some ways share similarities with the general population, making it difficult to detect them. There also are many different political, religious, social, and psychological motivations for terrorists to commit acts of terror.

In addition to studying terrorists and their motivations, you also examine terrorist groups, their evolution, and their distinct characteristics. Initially you are exposed to literature on a wide range of terrorist groups, including homegrown and international terrorist groups. However, as you progress through the course, you focus exclusively on Al Qaeda and its associated movements (AQAM). You focus on AQAM because it poses a significant threat to U.S. homeland security. As you study AQAM, you consider its political objectives, ideology, worldview, and terrorist activities. You also consider the current debate about its evolution and structure as a leaderless movement or a hierarchical organization.

The remaining weeks of the course focus on U.S. counterterrorism structures, policies, and strategies. You begin by examining the pre-9/11 approach to terrorism. Prior to 9/11, terrorism was viewed as a law enforcement issue rather than a homeland security issue. Therefore, the focus was on gathering evidence to capture and convict suspected terrorists of crimes they had already committed rather than trying to predict and prevent terrorist attacks. In addition, government agencies vital to the prevention of terrorism (e.g. FBI, CIA, FAA, etc.) lacked communication and collaboration. To round out your study of the pre-9/11 approach to terrorism, you also explore the 9/11 Commission Report. The 9/11 Commission was created in late 2002 to examine the circumstances surrounding the 9/11 terrorist attacks. In 2004, the Commission published a report that included missteps that occurred prior to the attacks as well as recommendations for future counterterrorism efforts. You explore those recommendations, their significance, and their implementation.

After the 9/11 Commission published their report, there were major changes made to national security laws and policies. Several legislative acts were passed to strengthen national security, ensure public safety, and prevent future terrorist attacks. You explore many of these legislative acts, including the Transportation Security Act, Homeland Security Act, Comprehensive Security Act, and Patriot Act, as well as their strengths and limitations. This provides you with foundational knowledge for a later week in this course, in which you examine the delicate balance between civil liberties and national security. It also provides you with foundational knowledge to complete the Final Project. For your Final Project, you are asked to develop a proposal on how to improve U.S. counterterrorism policy in a specific area.

The “lessons learned” and recommendations from the 9/11 Commission Report, and the laws and policies passed following 9/11, directly influence contemporary U.S. counterterrorism strategies. One area that received significant attention was intelligence. Information gathering is an important tool for predicting and preventing terrorist attacks and warning government officials and citizens of potentially imminent terrorist attacks. However, there are many challenges related to collecting intelligence and issuing warnings. In this course, you explore pre- and post-9/11 counterterrorism strategies and consider some of the challenges related to ensuring the effectiveness of counterterrorism efforts.

As you near the end of the course, you take an in-depth look at international law, its impact on the designation, detainment, and treatment of suspected terrorists, and the political, legal, and ethical issues related to the treatment of detainees. As you do so, you consider the complexities related to how to best manage suspected terrorists during times of war. You also reflect on the future of Al Qaeda in the coming years and consider how the U.S. might amend or adapt its structures, policies, and strategies to address the evolving terrorist threat.

Motivations of Terrorists

What motivates a man like Osama Bin Laden to encourage the use of indiscriminate violence to further his political objectives? How, if at all, do his motivations differ from those of terrorists of a previous generation, such as Carlos the Jackal? Although it is common, even stereotypical, to consider terrorists as all having similar view points, in most cases they are distinctly different. Goals and motivations vary at the organizational and individual levels. Socialization, revenge, and perceived injustice are all possible motivations, but, like causes of terrorism, there is no single explanation behind each act.
