Identify the case you will be using for the final project

In order to provide the appropriate intervention, social workers first need to have conducted a thorough assessment. Having a comprehensive understanding of a client and their social environment, including the influence of racism and ethnocentrism, allows a social worker to accurately identify the presenting problems and help the client develop goals to address their needs. Not completing a full assessment may result in inaccurately identifying the presenting problem and pushing goals on the client to which they do not agree. A good assessment is the best foundation for treatment planning.

In addition, evidence based practice requires social workers to use the best available evidence to inform their practice decisions. This assignment helps you prepare for the final project by beginning to identify the presenting problem and possible goals, as well as beginning the research process to inform your treatment recommendations when working with the identified client in a case study.

To prepare: Conduct research in the Walden library to locate at least 1 peer-reviewed scholarly resource(s) that could inform your assessment or intervention with the case you plan to use in your final project. (Using Dalia Case in the attachment).

    Dalia is a 14-year-old, biracial female of African and Irish American descent who resides with her parents in a middle-class suburb. She is the youngest of three children and is currently the only child remaining in the home. Dalia’s parents have been married for 25 years. Dalia’s father works in the creative arts field with a nontraditional work schedule that has him gone overnight and sleeping late into the morning. Dalia’s mother is an executive who works long hours. Dalia was diagnosed with sickle cell anemia in early childhood and was hospitalized multiple times. At present, Dalia’s health is stable with the last serious episode occurring 2 years ago.

    Dalia’s parents reported that until middle school, Dalia was an easygoing, good-natured youngster who enjoyed singing and participating in activities with her peers and family. Dalia denied any problems with drugs or alcohol but admitted to drinking with friends. Dalia described her family relationships as tense, stating, “My dad lets me do what I want” and “My mother is always trying to control me.” Dalia described her relationship with her older brother, who lives in another state, as “cool,” and her relationship with her older sister, a college sophomore, as “not cool.”

    Dalia’s parents sought out counseling services for behavioral issues on the recommendation of her school. The issues included argumentative behavior with authority figures, physical altercations with peers, poor concentration in class, irritable mood, verbal combativeness when confronted, truancy, and highly sexualized behavior with male peers. At home, Dalia had become more argumentative and physically threatening. Her parents had discovered that she and her friends drank alcohol in their home. Dalia’s parents also reported that she was up most of the night and slept most of the day. They also reported that her mood was highly irritable and that she was extremely impulsive. She had no interest in getting involved with any extracurricular activities, stating that those things are “corny and boring.”

    In the first meeting, Dalia and her mother both appeared agitated with each other and became argumentative when going through the intake information. Dalia quickly told me that she was not planning to talk about anything because this meeting was her parents’ idea. She stated, “I don’t have any problems, my parents do.” Soon into this first visit, Dalia blurted out that her mother was upset with her because she had just shown her a tattoo she had had done recently, purchased by using a fake ID. I acknowledged her news and asked if this was the way that she usually shared important information with her mother. Dalia shrugged and stated, “I don’t know. I figure I better her tell her now before she gets too busy.”

    I asked both Dalia and her mother what their expectations were for counseling and what each would like to get from these visits. Dalia’s mother seemed surprised and stated, “This is for her. She better change her attitude and start to focus on school.” I explained that often it is helpful to have sessions both individually and with family members. I pointed out that because family issues were identified it might be productive to address them together. Dalia’s mother agreed to attend some meetings but also stated that her time was limited. I was told that Dalia’s father would not be able to join us because he was never available at that time.

    Dalia and I began sessions alone, and her mother joined us for the second half. During the family sessions, we addressed the communication breakdown between Dalia and her mother and Dalia’s at-risk behaviors. Individual sessions were used to address her impulsive behavior and self-esteem issues.

    In individual sessions, Dalia talked about how the family had changed since her sister left for college. She said her parents stopped being present and available once her sister went away to school. She said she spent more time on her own and her behavior was under more scrutiny. Dalia also talked about her sister, describing her as an excellent student and very popular. She said her teachers in middle school would often compare Dalia to her sister, making her feel unsuccessful in comparison. During a family portion of a session, Dalia’s mother initially disagreed with Dalia’s point of view regarding how the family had changed, stating, “She’s just trying to trick you.” I encouraged them to discuss what was different about the family dynamics now compared to when the older sister was at home. We discussed how the family had changed through the years, validating both perspectives.

    In time, I was able to have Dalia’s father join us in some of the family meetings. He said he felt Dalia’s behaviors were just a stage and part of being a teenager. Dalia’s parents disagreed openly in our sessions, with each blaming the other for her behavioral issues. During these sessions, we addressed how they each may have changed as their children matured and left home and how this affected their availability to their youngest child. I helped them identify what made Dalia’s experience distinct from her siblings’ and examine what her high-risk behaviors might be in reaction to or symptomatic of in the family.

    In the course of the family work, the realities of being a biracial family and raising mixed-race children were also addressed. We discussed how the parents navigated race issues during their own courtship and looked at the role of acculturation and assimilation with their children in their social environments as well as respective families of origin. Educating both parents around race and social class privilege seemed fruitful in understanding distinctions between what they and their children may have faced.

    After 12 weeks it was agreed that therapy would end because Dalia would be starting high school and the family felt better equipped to address conflict. The family had made some changes with the household schedule that increased parent–child contact, and Dalia agreed to more structure in her schedule and accepted a position as a camp counselor in a local day camp for the summer. Termination addressed what was accomplished in this portion of therapy and what might be addressed in future counseling. The termination process included reviewing the strategies of conflict resolution and creating opportunities for family contact and discussion in order to reinforce those behavioral and structural changes that had led to improved communication and conflict reduction.

Results demonstrated that kids who received comprehensive evaluations had a greater degree of diagnosis for comorbidities. The authors examined the impact on parents who have a child with ADHD and comorbidities. Using the National Survey of Children’s Health dataset, they conducted a bivariate, multivariate, and descriptive analysis to look for associations between kids with ADHD and comorbid conditions and the strain on parents, social support, mother’s mental health, and local amenities. Results showed an increase in parental strain when caring for an ADHD child with a co-occurring condition. It also showed that lack of social support and lack of access to community amenities were predictors of increased parental strain. This study demonstrates the impact on the health of caregivers to ADHD children with comorbidities.

Radigan, M., Lannon, P., Roohan, P., & Gesten, F. (2005). Medication patterns for attention-deficit/hyperactivity disorder and comorbid psychiatric conditions in a low-income population. Journal of Child and Adolescent Psychopharmacology, 15(1): 44-56.

The authors examined the psychotropic medications usage of low-income kids who have been diagnosed with ADHD comparing those with and without comorbid conditions. The New York State Department of Health Medicaid Encounter Data System was used to extract information on 6,922 kids 3-19 years of age. A multivariate logistic regression was conducted to look at associations between ADHD with comorbid conditions and medication usage. Results showed the strongest predictors of medication use to be comorbid conditions and Social Security Income Medicaid eligible status. This study stimulates the question of the possibility for ADHD children with comorbidities to have treatment variations based on income status.

Rockhill, C., Violette, H., Vander Stoep, A., Grover, S., & Myers, K. (2013). Caregivers’ distress: Youth with attention-deficit/hyperactivity disorder and comorbid disorders assessed via telemental health. Journal of Child and Adolescent Psychopharmacology, 23(6): 379-385.

The authors examined the additional impact of comorbidities in kids with ADHD and the relationship to caregiver distress. Data was obtained from the Children’s ADHD Telemental Health Treatment Study for kids with ADHD in underserved communities. ANOVA and ANCOVA were used to look for associations between ADHD alone, ADHD plus one comorbid condition, and ADHD plus two or more comorbidities and caregiver distress. They found that parents of ADHD kids plus one or more comorbid conditions had higher levels of depression, stress, and strain compared to parents of kids with ADHD alone. This study reinforces the need to better understand the dynamics of ADHD and comorbidities and potential disparities.

Working With Clients With Disabilities: The Case of Valerie

Valerie is a 56-year-old, heterosexual, African American female. She receives Social Security Disability Insurance (SSDI) and works part time at a credit card company as a telemarketer. She currently lives in an apartment alone but receives home attendant services for 5 hours a day. She lost her left leg when she was hit by a car and has a prosthesis. She uses a walker or an electric scooter to be ambulatory but generally prefers the scooter. She is slightly overweight, which makes using the walker more painful. She has been prescribed Zoloft® (100 mg per day) for general anxiety and has been taking it for almost 3 years. Valerie has a history of drug and alcohol abuse, although she has been drug free for 15 years. She has a core group of friends she has maintained a relationship with over the course of her lifetime, and although she does not see them as often as she would like, she keeps in touch over the phone and through email. She has no criminal background.

Valerie came for services to address unresolved feelings related to an abusive marriage. She continued to be in contact with her ex-husband, John, although they had been divorced for almost 13 years. Valerie said that she and John had remained intimate since the separation and divorce and that John texted and called her to meet for sex. She felt torn because she believed no one else would want to date her due to her disability but also felt John was using her. She also stated that although he had stopped hitting her, he continued to be verbally abusive. She remained anxious and depressed and felt hopeless about the situation.

Valerie said John abused alcohol and began using drugs in the first few years of their marriage. Unaware of his illicit drug use, Valerie arrived home from work early one day to surprise him and found him using cocaine. John attacked her and forced her to use cocaine as well. She relented due to her fear of continued assault. An ongoing pattern of drug use and physical assault persisted throughout their marriage.

Valerie lost her left leg when she was walking across the street and was hit by a car, and she spent close to 9 months in the hospital and a rehabilitation program. She was fitted for a prosthetic leg and given an electric scooter through her insurance company, which allowed her to begin working part time at a credit card company when she returned home. John’s abusive behavior and drug use continued, so Valerie hid her paychecks, slowly saving her money until she had enough to leave. Eventually, she was able to rent a room. In addition, she was able to secure the assistance of a home health aide.

Valerie began individual and group sessions to address her feelings of depression and anxiety. I worked with her to set manageable goals to increase her independence in physical functioning and from her ex-husband’s controlling and abusive behaviors. Valerie and I agreed to use cognitive behavioral therapy to address her continued negative thought patterns that affected her behavior. Valerie shared many insights into her disability to help me understand how she felt in a world that was not very accessible. Through our meetings, I learned about the Americans with Disabilities Act (ADA) and how inaccessible buildings and programs affected her quality of life. We met once a week for 3 months, and I monitored Valerie’s depression through a baseline and then periodic administrations of a depression screen using the Beck Depression Inventory.

After 12 weeks, we decided together it was time for termination. She reported fewer episodes of anxiety and expressed feelings of hope for the future. She continued to attend the group sessions and found new friends who had become a support network for her. She had stopped seeing her ex-husband and changed her phone number to prevent him from contacting her.

Sociologist Erving Goffman developed the concept of dramaturgy, the idea that life is like a never-ending play in which people are actors. Goffman believed that when we are born, we are thrust onto a stage called everyday life, and that our socialization consists of learning how to play our assigned roles from other people. We enact our roles in the company of others, who are in turn enacting their roles in interaction with us. He believed that whatever we do, we are playing out some role on the stage of life.

    Example Research Proposal

    Introduction

    The concept of self-efficacy was introduced nearly 40 years ago. “Perceived self-efficacy

    refers to beliefs in one’s capabilities to organize and execute the courses of action required to

    produce given attainments” (Bandura, 1977, p. 3). Self-efficacy has been identified as an

    important construct for academic achievement in traditional learning environments for at least

    two decades. Zimmerman and Schunk (2003) go so far as to say that “the predictive power of

    self-efficacy beliefs on students’ academic functioning has been extensively verified” (p. 446).

    Its importance has been noted consistently through all levels of the educational process, with

    various student populations, and in varied domains of learning.

    While learner self-efficacy has a well-established literature base in the context of

    traditional learning environments, self-efficacy research related to learners in online and other

    non-traditional learning environments is relatively new. Hodges (2008a) has called for

    researchers to explore self-efficacy in online learning environments. Additionally, in terms of

    students’ self-efficacy beliefs toward academic achievement, “there have been few efforts to

    investigate the sources underlying these self-beliefs” (Usher, 2009, p. 275). The purpose of the

    proposed study is to investigate the relative strength of the four traditionally proposed sources of

    self-efficacy beliefs of students enrolled in a technology-intensive asynchronous college math

    college.

    Literature Review

    Self-efficacy beliefs have been found to be significant contributors to motivation and

    performance in academic achievement (Multon, Brown, & Lent, 1991), group functioning

    (Gully, Incalcaterra, Joshi, & Beaubien, 2002; Stajkovic & Luthans, 1998), health (Holden,

    1991), and sports performance (Moritz, Feltz, Fahrbach, & Mack, 2000). Research revealing the

    connection between self-efficacy and mathematics, the context of the proposed study, includes

    many cultures and levels of education (Malpass, O’Neil, & Hocevar, 1999; Pietsch, Walker, &

    Chapman, 2003; Randhawa, Beamer, & Lundberg, 1993; Stevens, Olivarez, Lan, & Tallent-

    Runnels, 2004) and continues to the present (Usher, 2009).

    Sources of Self-Efficacy

    Albert Bandura’s (1977) introduction of self-efficacy theory included the proposition that

    self-efficacy is derived from four principal sources: mastery experiences, vicarious experience,

    social persuasion, and physiological/affective states. These four areas are generally accepted in

    the literature as core elements in the development of self-efficacy beliefs, but an ordering of the

    importance of each of these sources is unsettled.

    Mastery Experiences. Mastery experiences refer to previous, successful experiences a

    learner has had performing a task. Successes build positive self-efficacy beliefs and failures

    undermine self-efficacy. If failures are experienced before a firm positive belief in one’s self-

    efficacy is formed, the creation of positive self-efficacy beliefs is more difficult.

    Vicarious Experience. Vicarious experience refers to one’s observation of a role model

    performing a task. Knowledge of how others have performed a similar task helps one determine

    whether or not a performance should be judged a success or failure. Surpassing the performances

    of others increases self-efficacy and falling below others’ performances lowers self-efficacy.

    Note the importance of the selection of individuals for comparison. Self-efficacy beliefs will

    vary depending on the abilities of those chosen for comparison, thus, models for comparison

    should be selected carefully (Wood, 1989).

    Social Persuasion. Social persuasion is commonly used due to the ease with which it can

    be dispensed. The believability of the persuader(s) is important in the use of social persuasion.

    The receiver must view the persuader as competent to provide meaningful and accurate

    feedback. Bandura (1997) cautions that verbal persuasion consists of more than flippant, off-

    hand comments of encouragement. Unrealistic comments from the persuader may mislead the

    receiver, which may decrease self-efficacy and diminish the belief in the persuader as one

    competent to evaluate the performance. “Skilled efficacy builders encourage people to measure

    their successes in terms of self-improvement rather than in terms of triumphs over others”

    (Bandura, 1997, p. 106).

    Physiological/Affective States. Stress, emotion, mood, pain, and fatigue are all

    interpreted when making judgments regarding self-efficacy. For example, someone may have

    prepared well for an exam, but upon learning of some unfortunate news, stress may reduce

    concentration, thus impacting performance on the exam. In general, success is expected when

    one is not in a state of aversive arousal (Bandura, 1997).

    Usher and Pajares (2006) summarize the inconsistent findings regarding the relative

    strength of each self-efficacy source well. They follow with the proposition that “exploring the

    predictive value of the sources of students’ academic self-efficacy beliefs and determining

    whether this prediction varies as a function of group membership such as gender, academic

    ability, and race/ethnicity is a matter of import” (p. 130).

    Methods

    Design

    The proposed study is quantitative in nature and will use a survey research design

    (Newman, 2011). Survey research falls into the non-experimental category of research designs.

    The survey questions use mostly ordinal scales and will result in numeric scores summarizing the

    extent of use of each source of self-efficacy beliefs as well as a score representing the level of

    self-efficacy held by each student in relation to the ability to learn math in an asynchronous

    learning environment.

    Participants

    Approximately 300 students in an asynchronous college algebra course offered at a large,

    state supported university in the mid-Atlantic region of the United States will be invited to

    participate in a survey. This is a convenience sample, and participation is voluntary, so the final

    sample size may be considerably smaller than the number of students invited. The course is

    delivered using an emporium format (Twigg, 2003) which is technology intensive. The students

    enrolled in the course tend to be engaged in academic majors that are not math-intensive. They

    may have a high degree of math anxiety or at least some negative feelings toward their math

    abilities. In addition, the emporium model may be an unfamiliar concept for them.

    Procedure/Measures

    This course is offered through the Math Emporium and has no traditional class meetings.

    After a brief, face-to-face, orientation meeting, students complete the course asynchronously.

    There are weekly deadlines for quizzes, and proctored tests are administered periodically.

    Students prepare for the quizzes and tests by taking advantage of various technology resources

    available to them online. Lesson pages serve as an online textbook for the course, short

    streaming video lectures are available on most topics, and an unlimited number of practice

    quizzes are available. For students who desire it, face-to-face interactions with assistants in the

    computer lab are available several hours each week. No appointment is needed for the face-to-

    face assistance.

    At the conclusion of the course, data will be collected using a web-based survey tool.

    Students who provide informed consent to participate will be given an ID number and survey

    access information. They may access the survey either in the Math Emporium or offsite through

    the internet. Specific instruments to be used are the Self-Efficacy for Learning Mathematics

    Asynchronously (SELMA) survey (Hodges, 2008b), a demographics survey, and the Sources of

    Mathematics Self-Efficacy (SMSE) scale (Lent, Lopez, & Bieschke, 1991).

    The SELMA survey is a 25-question survey constructed for use in college algebra and

    trigonometry courses offered in an emporium model. A validation study showed an internal

    consistency Cronbach’s alpha value of 0.87 (Hodges, 2008b) which is greater than the 0.80

    minimum level recommended by Gable and Wolf (1993) for instruments in the affective domain.

    The SMSE scale consists of four 10-question subscales designed to measure each of the

    four sources of self-efficacy: mastery, vicarious experiences, social persuasion, and

    affective/physiological state. In a validation study of the SMSE, Lent et al. (1991) reported

    internal consistencies of 0.86 for mastery, 0.56 for vicarious, 0.74 for persuasion, and 0.90 for

    affective/physiological arousal.

    Data Analysis

    To investigate the relative strength of the four traditional sources of self-efficacy beliefs

    of students in an asynchronous math course, analysis of variance (ANOVA) and multiple

    regression will be used. Scores from each of the four subscales of the SMSE will be used as

    predictors of the SELMA score. Bivariate correlations will also be examined. Significant

    correlations among the predictor variables may present a problem of multicollinearity. If

    necessary, additional statistical tests such as ridge regression (Joe & Mendoza, 1989; Kidwell &

    Brown, 1982) will be applied to solve this problem.

    Ethical Issues

    Participation in the survey will be strictly voluntary, and will not be tied to evaluation of

    the student’s performance in the course in any way. As a non-experimental survey study, no

    deception will be used. Signed informed consent will be obtained from those who wish to

    participate. Those who agree to participate may withdraw from the study at any time without any

    type of penalty.

    Confidentiality of participants will be protected by the assignment of ID numbers to be

    used on the survey documents instead of names or any other type of identifying information. A

    single copy of the list matching the ID numbers with participants’ names will be kept in a secure,

    locked location for a period of three years after the completion of the study. After three years, the

    list will be destroyed in accordance with the instructions of the Institutional Review Board

    (IRB).

    As a token of appreciation, all participants will be entered into a drawing for an Amazon

    gift card. The proposed amount of the gift card, subject to IRB approval, is $25. University

    facilities, including the computer lab known as the Math Emporium, its computers and a survey

    software program, will be used if this study is approved. This project will not receive any

    external funding from commercial or other sources, and no conflicts of interest are reported by

    the researchers.

    Conclusion

    Self-efficacy and its relationship to academic achievement in asynchronous online

    learning environments are only recently beginning to be researched (Hodges, 2008a). Given the

    growing prominence of asynchronous online learning, it is essential that we understand what role

    constructs such as self-efficacy play in these learning environments. The proposed study will

    address this need by using a survey research design. The surveys will provide data on the four

    sources of self-efficacy which will serve as predictors of students’ self-efficacy for learning

    mathematics in an asynchronous online setting. A multiple regression model using the four

    predictors with the SELMA survey score as the dependent variable will indicate how much each

    source contributes to self-efficacy.

    The results of this study are expected to be important to instructional designers and

    educational practitioners who either currently use or are considering using an emporium model,

    as they will give indications of which elements of the asynchronous course design should be

    emphasized to best promote students’ self-efficacy relating to the subject matter. An expedited

    review of this proposal by the IRB is requested for approval to begin this research as soon as

    possible.

PUBLIC POLICY ISSUES AND TERMINATION

The public policy exception to the employment-at-will doctrine provides that employees may not be terminated for reasons that are contrary to public policy. Public policy originates with legislative enactments that prohibit, for example, the discharge of employees on the basis of disability, age, race, color, religion, sex, national origin, pregnancy, union membership, and/or filing of safety violation complaints with various agencies (e.g., the Occupational Safety and Health Administration). Any attempt to limit, segregate, or classify employees in any way that would tend to deprive any individual of employment opportunities on these bases is contrary to public policy. Brandi Cochran, who had been a presenter on “The Price Is Right,” said she was fearful of losing her job if she got pregnant. She was right: she lost her job. She sued the game show, claiming wrongful termination based on her pregnancy. The jury awarded her more than $7.7 million in punitive dam-ages. Discrimination based on her pregnancy was both illegal and unfair. If a woman is temporarily unable to perform her job due to a medical condition related to pregnancy or childbirth, the employer or other covered entity must treat her in the same way as it treats any other temporarily disabled employee. For example, the employer may have to provide light duty, alternative assignments, disability leave, or unpaid leave to pregnant employees if it does so for other temporarily disabled employees.5Public policy also can arise as a result of judicial decisions that address those issues not covered by statutes, rules, and regulations. “[I]t can be said that public policy concerns what is right and just and what affects the citizens of the state collectively. It is to be found in the state’s constitution and statutes and, when they are silent, in its judicial decisions.”6Public policy favors the exposure of crime, and the cooperation of citizens possessing knowledge thereof is essential to effective implementation of that policy. Those persons acting in good faith, who have probable cause to believe crimes have been committed, should not be deterred from reporting them for fear of unfounded suits by those accused.7In those instances in which state and federal laws are silent, not all courts concur with the use of judicial decisions as a means for determining public policy. A California court determined that a public policy exception to the employment-at-will doctrine must be based on constitutional or statutory provisions rather than judicial policy making.

As health insurance providers shift from fee for service to value based payment models, the importance of adapting to the changing industry landscape is necessary for any healthcare provider to thrive. This includes the implementation of electronic medical records, strategies for managing bundled payment participation, and initiating preventative care and cessation measures among their patients. Employees are often on the front line of these changes, meaning their day to day functions as well as the organizational structure could significantly be impacted. It is important that leaders not only lead process change but also lead their people through those times of change (Selivanoff, 2018).

As Ted Powell noted, there are four stages of change that employees will progress through; denial, emotional, acceptance, and commitment. From an intellectual standpoint, leaders should explain the necessary steps and reasons for change with supporting data or industry examples. This will ensure the employee that what is being asked of them is possible. In addition, the leader must appeal to their emotions. Managers should frame conversations so the employee feels the change is in line with their needs or values (Selivanoff, 2019).

Change is inevitable. However, there are many who become extremely uncomfortable when asked to step away from their comfort zone and can become defiant; often due to fear of the unknown, thus blocking the necessary movement forward.

Powell (2014) describes the four typical stages employees may experience as a result of a large organizational change:

  1. Denial:”If it ain’t broke, don’t fix it.” There’s no need for change.
  2. Emotional: Problem-identifying mindset. Low morale and negative emotions run rampant.
  3. Acceptance: Problem-solving mindset. An optimistic attitude starts growing.
  4. Commitment: Increase in morale and productivity. We’ve overcome the worst together and are stronger for it.

I felt that the overall take home message from the article “The Four Stage of Organizational Change” was that in order to get anything accomplished, you need buy-in from your staff. To do that, you need patience, communication, and resources in place to support them. Broder (2013) did a great job laying out five steps necessary to get “buy-in” from employees in order to get anything accomplished.

  1. Lay out the vision. Clearly state what and why it is important to change.
  2. Personalize tasks. Assign tasks with measurable goals that play to their strengths in order to motivate them to achieve success.
  3. Follow-up. Stay connected during process to trouble-shoot as needed and let them know you’re willing to help them through the needed changes.
  4. Nip resistance in the bud. One rotten apple can spoil the bunch so address negativity immediately.
  5. Be prepared to change the change. Solicit feedback from employees and fine-tune the plan by making corrections to process as you go along

I think the feelings some employees express as a result of change are similar to those when experiencing grief. They’re somewhat similar in that the employees may feel as though they’re losing something familiar and are grieving the loss, being forced to embrace something new that they did not choose for themselves. Ultimately, you cannot rush anyone through any of the stages – they will have to work through them at their own pace.

One of the tools often used to begin addressing the need for change in any organization is the ‘survey tool’.

Select a data collection questionnaire or tool that has been designed by a healthcare organization to obtain feedback on their overall effectiveness or the effectiveness of a particular information system within the organization. These may be questionnaires that are administered online or in paper form.

There are several ways to find such questionnaires. For example, they may appear in a journal article reporting on their use. To locate such instruments, search the Library business database on business evaluation topics that may be of interest to you or on the names of particular organizations that might be of interest to you. Do a review of the Web and find a survey tool that is used by health organization(s).

Spend some time reviewing the mission and goals of the organization you select so that you understand what they are trying to accomplish and why the questionnaire was used for evaluation. The names (and sources) of the instrument and the company in which it has been used should be included in your work.

Analyze the instruments to determine the following:

One of the unique features of the healthcare industry is that the patient or customer usually does not pay for their services directly.  Payment is made by a third-party payer and how and by which one often influences which services are provided and by whom (Gapenski, 2018). Third party payers include:

  • Private Insurers – Blue Cross/Blue Shield, commercial insurers such as AETNA and Humana or Self-insurers such as large state programs or self- insured providers.
  • Public Insurers – Medicare or Medicaid

Industry wide increasing costs of care drove the demand for cost controls – hence the creation of managed care.  “Most definitions characterize managed care as a system that integrates the financing and delivery to appropriate medical care by means of the following features: contracts with selected physicians and hospitals that furnish a comprehensive set of health care services to enrolled members, usually for a predetermined monthly premium; utilization and quality controls that contracting providers agree to accept; financial incentives for patients to use the providers and facilities associated with the plan, and the assumption of some financial risk by doctors, thus fundamentally altering their role from serving as agent for the patient’s welfare to balancing the patient’s needs against the need for cost control – or  as Mechanic put it succinctly, moving “from advocacy to allocation” (Iglehart, J. 1992 p. 742).

Managed care gave rise to several new types of insurance deliverables.  Health maintenance organizations (HMOs) are based on the premise that the traditional insurance-provider relationship incents treatment of illnesses, but not prevention of them (Gapenski, 2016).  “By combining financing and delivery of comprehensive healthcare services into a single system, HMOs theoretically have as strong an incentive to prevent illnesses as to treat them “(Gapenski, 2016, p. 48).   However, requiring the assignment of a primary care physician and limited providers to those within the HMO’s network make them less than desirable to many patients.     Preferred provider organizations (PPOs) are a hybrid of HMOs and traditional health insurance.   They do implement many cost cutting measures like HMO’s, but don’t restrict the patient to one physician, or only physician that are in network (Gapenski, 2016).  But regardless of the payer or payer type for a particular healthcare service, only a limited number of payment methodologies are used to reimburse providers (Gapenski, 2018, p.52).  They fall into two categories – fee-for-service and capitation.

Fee-for-service charging has been the dominant method for many years, and most patients think of their care in these terms – the more the physician does, the more it will cost.  Three fee-for-service methods include (1) cost-based reimbursement where the payer agrees to reimburse the provider for the allowable costs incurred to provide the service; (2) charge-based reimbursement, when payers pay billed charges based on a provider rate schedule called a chargemaster; or (3) prospective payment where the payer sets the rates (i.e. by procedure, by diagnosis, per day or bundled) before the service is provided  (Gapenski, 2016).

Furthermore, units of payment are defined as:

  • Per procedure – a separate payment is made for each procedure performed.  Most commonly used in the outpatient setting, the concern with this payment type is the high administrative costs with complex diagnoses (Gapenski, 2016).
  • Per diagnosis – the provider is paid a rate based on the patient’s diagnosis.  The practice was pioneered by Medicare in its diagnosis-related groups (DRGs) used to define hospital inpatient reimbursement.  The risk here is if the diagnosis is very complex.  Even if the provider requires more resources to treat the patient, they will only be reimbursed the pre-set diagnosis rate (Gapenski 2016).
  • Per day (per diem) – the provider is paid a fixed amount for each day of service provided.
  • Bundled – payers make a single payment that covers all services delivered in single episode regardless of the number of providers involved (Gapenski, 2016). An example of bundled pricing is maternity or obstetrical care.  A bundled or global rate may include the physician’s office visits, lab work, or other testing — all billed and paid together.
  • When reimbursing for care under the capitation model, the provider is paid a fixed amount per covered life period regardless of the amount of services provided.   Used primarily by managed care plans, both providers and insurers are tasked with enhancing quality while constraining costs (Gapenski, 2016).

Fee-for-service has fallen out of favor.  The long-standing fee-for-service model is methodically being replaced by a value-based payment system that rewards providers based on efficiency and patient outcomes rather than volume, according to local stakeholders.    “The fee-for-service model is easier to do. You assign a payment level and pay. But it doesn’t really align incentives very well,” said Ward Sanders, president of the nonprofit New Jersey Association of Health Plans. “We want to get to a place where we’re rewarding providers not for how much care they provide but for the outcomes and how well they provide care” (Vecchione, A, 2018).

While reviewing the managed care contracts we find two main types; the health maintenance organizations (HMO) and preferred provider organization (PPO).  The HMO only has a select few providers and individuals are assigned a primary care manager.   In the PPO plan patients are not assigned a set provider and they have a greater selection of providers to choose from. To increase productivity and cost efficiency they recently added a managed fee for service plan (Gapenski, L. C. Reiter, K. L. p. 48, 2016).

There are two main categories of reimbursement methods for managed care, the reimbursement methods are fee-for-service and capitation.  The fee-for service has three main categories cost-based, charge based, and prospective payment (covers per and bundled categories). Service fees are given to providers based on type of services provided. The incentive portion of cost based is that providers are guaranteed payment. In charge-based reimbursement individuals are billed for services that the provider sets.  Currently with charge-based, companies negotiate or discount charges if they have a large group of participants.  The prospective payment plan individuals are made aware of the rates for payment prior to the services being given.  This does not rely on allowable costs or charges unlike the cost-based and charge-based.  In the capitation reimbursement providers are given a fixed rate over a set time regardless of what service the provider gives (Gapenski, L.C., Rieter, K. L. p. 52-54, 2016).

Cost-based incentive would be for the provider to accrue more costs. A risk is providing unneeded services. For the charge-based an incentive would be to apply higher charge rates and the risk is that there may be a limit on how high providers can go.  Procedure providers would need to increase the number of procedures that have a high profit.  The risk is reimbursement can vary based on the actual cost of a procedure.  Per diagnosis, increase the profit by choosing the diagnosis code with the greater profit. However, some diagnosis cost less than others.  As for the per diem providers need to increase the amount of days patients will stay at the hospital to increase profit. On the other hand, risks would be shorter stays have less reimbursement.

Bundled providers would need to increase number of cases/episodes.  Risk is they can be “unbundled” which decreases amount of reimbursement.  Capitation would to increase health and decrease utilization to gain a profit.  The risks are only treatments that are medically needed would be done (Gapenski, L.C., Rieter, K. L. p. 55-57, 2016).

Since capitation is fixed an incentive for providers is to minimize costs to gain more revenue as well as provide services that reduce cost.  With capitation providers may only treat patients that are low risk.   In the fee-for-service reimbursement providers can increase services administered to receive a higher reimbursement.  In bundled reimbursement providers a given an amount for all services given for one case, this encourages providers to give quality care and keep the cost for each case low. (Barnum, H., Kutzin, J., Saxenian, H., p. 6-7,10-12. 2006).

Each type of pure reimbursement provides some measure of risk selection and incentives.  Global there is incentive to reduce cost, increase services and have little risk selection.  Fee-for service providers will have a strong incentive to increase service, reduce costs with some risk selection.  In capitation providers want to increase services and has a lot of risk selection.  With bundled providers will reduce cost, increases caseloads, and has a high chance of risk selection. Think of the many names and labels you may have heard to describe persons with disabilities and those that are currently socially acceptable. The changing monikers given to those with disabilities are evidence of the continual negotiation of the society who labels and those who are so labeled to define what disability is and who is disabled. What do these shifting labels suggest about the social construction of disability?

Society is inconsistent in its treatment and protection of the rights of individuals with disabilities, creating a situation that contributes to marginalization that can complicate other forms of marginalization and oppression. Consider that being labeled with a disability can be simultaneously something to be fought against because of the stigma it entails and fought for because of the access that it grants to social services that meet basic medical needs, aid economic survival, and improve access to education that society can otherwise deny.

I think the size of my business would play a huge factor in whether I utilized an ERP system or not. A business with 10 or so employees, I would probably stick with Microsoft Office. Long-term care services are services that provide help and assistance to patients that need medical care for an extended period of time. Long term care can be obtained for elderly patients who may no longer be able to perform ADLs without assistance or for a individual recovering from an injury and is not able to care for themselves during their recovery time. There is a multitude of uses for long term care services as well as places that provide them. Many rehabilitation facilities, assisted living facilities, and even home health care provide long term care for their patients.

Scenario: A company called Colony Nursery and Landscaping opened a new store located a few hundred miles away from its original location. The company wants to implement an award system that awards their customers with points whenever customers make a purchase, but the two stores are not able to share information. Colony Nursery and Landscaping will need to implement an enterprise resource planning (ERP) system that will solve the information silo problem by collecting and making this user data available. Colony Nursery and Landscaping is hoping that by providing customers with this award system, they will be able to maintain competitive advantage. Colony Nursery and Landscaping cannot afford to purchase, develop, or maintain this system on-site, so they are investigating cloud solutions.

In addition, for many organizations, Colony Nursery and Landscaping included, information silos make it difficult to tap into needed information. Genetic Programming

The first portion of the assignment is to implement the GP for symbolic regression. I strongly encourage you to use an already created GP system for this assignment (although, if you want, by all means make your own from scratch). Two popular ones are ECJ1 (Java) and DEAP2 (Python), but feel free to use any system you like, but please do check with me if you want to use one implemented in a language other than Java, Python, C, C++, C#. If you want, you can use mine3, but note that it will only work on symbolic regression and it is not that well documented (but it is very good at symbolic regression).

Any academic misconduct will be investigated fully and I will push for the maximum allowable penalty.

I have provided you with a collection of data in CSV format. For the most part, this data was generated by me by randomly generating data points, pumping it through a function, and then adding a little bit of noise to the output. See if you can reverse engineer the functions I used to create the data. All the data is formatted such that the first n-1 columns are the independent variables and the nth (last) column is

the dependent variable. For example, in ‘d1.csv’ there are two columns. The first column we will call and the second we will call y. We need to find some function of that will predict y. So, y ≈ f(x). If we had 3 columns, we would want z ≈ f(x,y). Note I have approximately equal to because you may not find the exact functions, but you will likely still get a close approximation. Ultimately, your goal is to use symbolic regression to try to find f.

Assuming you get everything working and you don’t have any serious problems, you will automatically get +10 on your assignment.

Typed Problem

If you choose to do a typed problem you can gain an additional +4 points. You will have to pick your own problem, so go find your own data to play with. I recommend checking out the UCI machine learning repository4. You must demonstrate to me that you have it working and that the problem is sufficiently challenging enough (and typed) to obtain the additional points.

Modifications

If you implement modifications for +1 each (max +2), be sure to make them obvious to me. For this assignment you may not use elitism as a modification. I do not care if you go out and find modifications or if you invent your own, just be sure to convince me that you deserve the extra points. If your improvement is not obvious to me, or if I deem it as not significant enough, you will not get the marks. If you choose to do the writeup then explain these modifications within the writeup. If you do not do the writeup, at least include a text file with a description of your modifications so I know what they are.

You can obtain a maximum of +9 for doing a writeup with +5 being from the base report. WARNING: The writeup is not trivial to do well and will take some time to write. This writeup will be marked more qualitatively by a marker. There is no precise best way to structure a writeup and it is difficult to know exactly what should be included. A portion of these marks will be dedicated to prose, understandability, continuity, spelling, grammar, content, and effectiveness. You can find an example of an article I wrote for publication this year to get a sense of what is good, but there is no one right way to do it and I would not recommend making your report look like mine (I’m simply giving it to you for an example, but you can find a lot more online). Below is a list of ideas on what to include:

State what the metaphor refers to. What does it mean?

We all have phrases we use from time to time that are actually metaphors.

For example, I am sure you are familiar with some of the following:

It’s ‘raining cats and dogs’

He has a ‘heart of gold’

Don’t ‘shoot the messenger’

That’s a ‘low ball offer’

For this assignment I would like you to do the following:

1) Choose a metaphor that is commonly used in American English and use it in a sentence. (If you would prefer to select a metaphor in another language you can, you will just need to say which language, translate the phrase, say what it means, and be able to describe the etymology/origins of the metaphor).

2) State what the metaphor refers to. What does it mean? (Try your best to do this without using another metaphor, although you may find this very difficult).

3) Research the origins (i.e. the etymology) of this term. How and when did this come about? You will be doing informal internet research to do this, just provide the link you used at the bottom of your submission.
