Patient outcomes and the fulfillment of care goals is one of the major ways that healthcare success is measured. Measuring patient outcomes results in the generation of data that can be used to improve results. Nursing informatics can have a significant part in this process and can help to improve outcomes by improving processes, identifying at-risk patients, and enhancing efficiency.

Purpose: 

The purpose of your rhetorical analysis is to analyze another writer’s argument.  Of course, your essay should address what the writer has written, but the emphasis of your rhetorical analysis essay should be a close examination of how the writer has presented an argument. Your purpose is not to argue with (or state your agreement with) the writer’s position. Your primary purpose is to analyze the strategies and features another writer has used to be persuasive. Do not be diverted from your primary task: to demonstrate that you have uncovered interesting, important things about the way the author’s argument has been presented.

Process:

First, you need to choose an article to analyze. Consider the following options from Writing Memphis:

        Patient: TJ, a 32-year-old pregnant lesbian being seen for an annual exam.

As future nurse practitioners, we will be providing care to patients with diverse backgrounds. Diversity means every one is unique and has individual differences in relation to race, ethnicity, gender, sexual orientation, socio-economic status, age, physical abilities, religious beliefs, political beliefs, or other ideologies (Queensborough Community College, 2018, para. 1). It is important for clinicians to understand that when caring for diverse patients, we must provide acceptance and respect so that a good ‘relationship’ can form, and optimal health care can be provided to that patient.

Week Two Patient- TJ

Exam Notes- (Focused for Week 2 Discussion)

TJ is a 32-year-old Caucasian female

CC: Presents for an annual wellness exam. She is currently 24 weeks pregnant G1:P 0: Ab 0 and is receiving care from an obstetrician. Pt received sperm from a local sperm bank.

HPI: Unremarkable; however, patient states she started having thick white, itching, slightly foul-smelling vaginal discharge one week ago. Pt denies current feelings of depression or anxiety.

Current Medications: Prenatal vitamins and acetaminophen PRN for aches and pains

Allergies: NKDA, no food or Latex allergy

Social HX: Pt attended graduate school and is currently is an attorney for a large local firm practicing family law. Pt states she has “good health insurance” and is “very financially secure” at this time. Pt states she has a partner, DR, who is currently living separately from the patient. TJ lives in a large single-family home 10 minutes outside of the main city of Boise. The patient has one dog and no other pets including cats or exotic animals. The patient states they are planning on living together in her home after the baby arrives. TJ states she is in a monogamous relationship with DR and believes that her partner is monogamous as well. The pt. states she and her partner mutually chose the donor sperm, and her partner was present for the IVF procedure. This was her second attempt at conceiving a child and both are very excited about expecting a child. TJ states she and her partner attend all scheduled prenatal appointments and they are looking forward to attending parenting classes specially focused on lesbian mothers-to-be later that month. The patient states that she feels “very safe” in her relationship with her partner. Pt denies current or past illicit drug, prescription drug, alcohol, tobacco, or nicotine use. Pt states she is not exposed to second-hand smoke or other chemicals at home or work. Pt states she is religious and that she was raised Catholic; however, she has not affiliated anymore with the church since her announcing she was a lesbian to her friends from church. She felt ostracized immediately after her disclosure and she has not attended a church of any type since. Pt states that she “still has faint in God” and “does not need a building to pray and have Him hear her”.  Pt has one sister, Anne, who she is close to and says Anne is supportive of her lifestyle and her pregnancy. Pt states her mom lives in another state and is supportive to the patient as well but “not as much as my sister is”. Pt further states she has two close friends from college who are also very supportive to her.

Family Hx: Strong family history of diabetes. Mother still alive and healthy. Father passed away from diabetes-related complications at the age of 53. Sister –hx.  type 2 diabetes and obesity.

12.  Are you keeping all scheduled and required OB appointments?

 

            Nurse Practitioner Sensitivity

According to the National LGBT Health Education Center (2016), lesbian, gay, bisexual, transgender (LGBT) patients have reported that health care providers have: used excessive precautions or refused to touch them (11%),  blamed them for their health status (12%), or used harsh or abusive language (11%) during health exams (National LGBT Health Education Center, 2016, p. 10). It is because of negative experiences and interactions with the health care system that lesbian women do not obtain the necessary health care, including preventative care, that they need (Gregg, 2018, p. 42). Because my patient is a lesbian, I would be mindful in delivering culturally sensitive care to her. I would utilize terms and pronouns appropriate to the patient, and if I were to inadvertently use the wrong ones, or make the patient feel uncomfortable, I would promptly apologize and state I did not mean to be disrespectful.

Unfortunately, LBGT patients have difficulty finding health care providers where they feel accepted, understood, and do not fear discrimination, and many will intentionally seek out care from practitioners who are familiar with this health population (Landry, 2017); therefore, I would make every attempt to create an environment that displays concern, inclusiveness, and respect and obtain further education and training focused on this patient population. I would also incorporate the RESPECT model in my care of diverse patients such as TJ. The RESPECT model stands for Rapport, Empathy, Support, Partnership, Explanations, Cultural Competence, and Trust and can help a clinician remain effective with communication and in providing patient-centered care (Ball, Dains, Flynn, Solomon, & Stewart, 2019, p. 31). I believe that LGBTQ patients deserve the same level and quality of care as heterosexual patients, and I personally will do everything in my power to ensure that they receive it.

  • The hospital urology department wants to establish a sexual dysfunction clinic. The department head wants to get an estimate of the number of men ages 35 to 60 in the community suffering with some form of sexual dysfunction.

However, the authors stresses that the erroneous administration of these drugs that are meant to cure the Leukemia, they can turn out to be a poison in the body which facilitates the advancement of the disease in the body and eventually leads to coma and consequently death.

From the above articles, various variables were identified which includes, the patients served between 4AM to 8AM experience higher rates of erroneous medication which is a time dependent variable. The second variable is on the medication ordered by practicing nurses, with resultant observation that it is very high more especially during weekends. This is an independent variable. The year of the trainee also means that a dependent variable in the last first year trainees committed many erroneous administrations of medication compared to final years trainees.

Scenario Mei

Mei is 10 months old and has been admitted to the paediatric ward in a regional hospital with her mother Morgan, who will be ‘rooming in’. Mei was referred by her GP who was concerned about Mei’s poor weight gain and increased lethargy.

Social History

Morgan is a single mother and Mei is her only child. Morgan has poor family support – her mother lives interstate and her father died when Morgan was a child. Morgan has not worked since Mei was born and misses her work colleagues.

Medical History

Mei was born at 40 weeks’ gestation following a normal vaginal delivery. There were no complications at birth and her birth weight was 3,200 grams. Mei was breastfed until 6 weeks of age. Mei did not gain weight during this period and would vomit after every feed. On the advice of the child health nurse Morgan started Mei on a formula recommended for babies with reflux. Mei did not gain weight and continued to vomit during and after every feed with regular periods of diarrhea.

At 6 months old Mei weighed 4,600 grams. The GP thought she may have a lactose sensitivity and suggested lactose free formula. Morgan again changed Mei’s formula. However, Mei continued to vomit during and after every feed. She has started on solids, but Morgan reports she isn’t very interested in food and continues to vomit after dairy products, biscuits, bread and packet baby foods. Her weight gain is very slow and despite regular check-ups with the GP and child health nurse her weight and energy levels have not increased. Mei is up to date with her immunisations.

Progress Notes

Mei is alert and bright eyed, but tires easily. She smiles and laughs with her mother and is verbalising, although does not yet have any words. Mei weighs 5,200 grams.

Mei has been diagnosed with a ‘cows milk protein allergy’. She will be commenced on prescription formula and requires a nasogastric tube to be inserted for top up feeds.

Morgan is angry about Mei’s diagnosis. She is defensive about Mei not gaining weight and blames nurses and doctors for not doing enough. Morgan has expressed frustration at the length of time it has taken to get the correct

Suicide is more than a private act amongst oneself. The sociological imagination helps us place seemingly personal troubles, such as loosing one’s job or feeling like committing suicide, into a larger social context, where we can distinguish whether and how personal troubles may be related to public issues (Kendall, 2018). It helps us take personal troubles, that are happening worldwide, and make them into public issues. For example, in New Delhi, India, a new economic boom has increased suicide rates in the 15-29 age category, especially high among those living in the wealthier areas. Many people would read about an economic boom and automatically think about how the civilians living in that area must be living a “good” life, when in reality, it is causing a major public issue behind the scenes. The result? Intensified job anxiety, higher expectations, and more pressure for individual achievement (Kendall, 2018). The sociological imagination is the reason today that we are able to take personal troubles, such as suicide, and link them to many different public issues, figuring out what the cause is. It has helped us focus more on the social problems causing individuals to act out rather than blaming individuals for creating their own problems, making them act in such ways. Many sociologists have also used the sociological imagination in a way to explore the relationship between suicide and today’s society, creating theoretical perspectives. One perspective that was applied to suicide is the Symbolic Interactionist Perspective which focuses on studying at a microlevel, analyzing people’s face-face interactions and the roles they play in society. From this point of view, a suicide attempt may be a way of garnering attention-a call for help- rather than ending ones life (Kendall 2018). Social forces influencing youth suicide trends starts with what one see’s, hears about or is exposed to. About a third of respondents reported hearing about cutting from an outside source, such as books, magazines, TV, or friends. In some instances, it was picked up from friends who were engaged in the practice. These findings suggest that some self-injurers do in fact learn the practice from others ( Taylor & Ibañez 2015). In today’s society I have observed many different things when referring to suicide. I believe that there are still problems with people blaming an individual instead of trying to solve the issue, whether it is personal or public; I have also seen many areas, globally, attempt to reach out and help suicidal people, making them feel like they have another option, another reason to continue living. Overall, the sociological imagination has helped us look deeper into the causes of suicide, analyzing the individual at a micro and macro-level, which has helped us realize the private issue is more of a public issue in todays society.

Hello, Sameera! I loved your interpretation of C. Wright Mills sociological perspective. I also agree that a problem often seen is blaming. I’d personally have to say blaming is another form of and/or can lead to shaming which is a big concern for me. An approach I’ve recently learned about that can help minimize such acts are social support as introduced in the functional perspective of sociological imagination! Like any plan, it could be disrupted, leading to a disorderly system, so I encourage anyone dealing with their own “private affairs or distresses” to continue to seek help and guidance and to be reminded that their problem is not being faced by only them but others too (global interdepence—a relationship in which the lives of all people are closely intertwined and any one nation’s problems are part of a larger global problem).

Sameera, I agree that personal troubles refer to the problems affecting the individuals, that the affecting individual, including other members of the society, typically will blame on the individuals own personal and moral failings. As you have said, blaming is a form of rejection that society uses to reject a person who made mistakes or failed at something. Instead society should do everything to still accept that person and help pick them back up. Just like Amber said in her reply, there are many things in society that will help an individual. Social Support definitely is a big one. Social issues are the main reasons why individuals commit suicide. When society makes you feel like you belong, you get a since of being wanted. This in turn gives that individual a since of purpose. Appraisal support is another key concept that should be followed. Having another person to get reasonable advice from to help in a certain situation is very beneficial. I agree that the Social Imagination helps us to get a bigger picture of what may cause suicide and being able to look at the smaller details, which help us to realize that an individual issue is more of a public issue.

One would think their struggles are supposedly lived alone and told they are brought on because of their own personal actions and decisions. C. Wright Mills proposes those thoughts to be disprovable through a systematic study called sociological imagination. It is simply not ‘you vs. the world’, but you AND the world. This perspective gives one the ability to differentiate experiences of oneself and societal molding and to see private affairs like financial instability or unemployment are caused by social causes like environmental concerns or even poverty. Social Imagination is constructed of research methods (e.g. surveys, questionnaires and interviews) and theoretical perspectives such as the functionalist perspective and symbolic interactions which views society on a microlevel.

Global interdependence—a relationship in which the lives of all people are closely intertwined and any one nation’s problems are part of a larger global problem (Kendall, 2018)

Mills points out that your problem is faced by others but even so your simple daily activities like jeopardizing study time to scroll on a social media platform and like pictures. (Examples of Sociological Imagination, n.d.) Sociological imagination, encourages one to think outside of their norm, to remove themselves and think beyond in certain approaches, global for example. Society removes individualistic decision making. One might think that their subconscious decision to take a certain route over the other or choice to purchase a specific brand instead of the other is commonsense, almost like an un-doubtly right decision that’s been shaped by past experiences. C. Wright Mills deems “commonsense” not commonsense at all but rather myths. (Kendall, 2018 p.6) For example, memes created by users on the internet and shared by thousands a day can be said to be agreeable or felt need to be. They encourage groupthink and could produce harmful outcomes like suicide. Memes or rather social media ultimately, are big social forces influencing suicide trends in the United States. Social media includes any coverage that can be seen, heard or experienced like tv shows (copycat suicides), the news, and peer pressure from (cyber)bullies or friends who’ve attempted themselves. Social media in the U.S. even effects our food and drinking consumption which becomes social rituals that places more importance on the symbolic value of a cup of coffee or tea than the food itself. A cup of tea could be more than for a health benefit but rather social interaction or coffee for its drug effects on the brain rather taste. (Crossman, 2019)

Is the saying, “money can’t buy happiness” true? The economic boom in Dehli, India proves it to be. The suicide rates in India are highest amongst the rich, in ages ranging from 15 to 29, as well the well educated. (NDTV.com, 2012;Lancet 2012; Kendall, 2018 p.7) It is understandable that with wealth and proper education more opportunities are introduced, however, so are job performance anxieties, mental and health issues (e.g. sleep disorders, depression, heart issues) and weakened relationships with people because of moving. When people move because of an increase of income, they lose social support which is expressed to be very important in the functionalist perspective of sociological imagination. The functionalist perspective, when disrupted leave people to question their lives and their ability to live it. One study that examined the functions of social support in reducing or preventing suicidal ideation in Air Force personnel during U.S. combat operations in Iraq and Afghanistan found that all forms of social support were not equally important in protecting individuals against suicidal thoughts or actions. Esteem support played a big factor in the severity of the Air Force personnel’s thoughts of suicide. (Kendall, 2018 p.15)

Sociological imagination helps identify the causes (and indirect preventions) and solutions by placing flaw in public issues rather than a person’s life’s decisions. Shame is a factor I’ve observed that results in suicide and many seemingly private acts like it. Others shame people into losing their interests and rob them of the fact they are human. This shame tactic is often seen with teen pregnancies, addiction, homelessness, incarceration and even simply music taste. Resources for help and guidance in certain communities are limited or not utilized as a result of self-shame or fear of being ridiculed.

Overall, the sociological approach has allowed for us the opportunity to look more in depth to an act deemed private. It allows us to take the action of an individual that would other wise be classified as personal and relate it to and identify society’s role in that action. We are able to then use theories and perspectives to help further understand an individual and find solutions to help.

Poverty from a sociological stand point can be interpreted in many different ways. Two main ways are; someone living in poverty brought it upon themselves or someone is living in poverty due to the society they live in and social forces around them. Generally speaking I truly think linking poverty (a personal trouble) to public issues is a hard connection to make.

Think of it this way, you can attempt to link poverty to unemployment issues globally but; not everyone who is unemployed is poor and not everyone who is poor is unemployed. Poverty is one of those things that is not fully understood yet due to the many different people/social classes dealing with the issue.

I do believe that there are some social forces and public issues that can be linked to poverty, especially those who are dealing with it while employed. For example, you have a single mother who is living in poverty while working a full time job but she’s only being paid minimum wage and she has to provide for herself and her child. With that example, we could definitely link minimum wage being too low (a public issue) to poverty (a personal trouble).

Although looking at poverty from a sociology stand point can be difficult, there are some ways you can look at it from the perspective of linking a personal trouble to a public issue.

He helped individuals see the relationship between personal experiences and the larger social world. Suicide is more of an individual act of oneself that may be the result of personal troubles or social issues. Personal troubles are private problems that affect individuals and the networks of people with whom they regularly associate (Kendall, 2018, p. 6). Today, there are many trends in different countries all around the world that influence the current rise of youth suicide. One example, in New Delhi, India, suicide rates are highest in the 15-29 age category and are especially high among those living in the wealthier and more educated regions of the nation (NDTV.com, 2012;Lancet 2012; Kendall, 2018, p. 7). One might believe that in the world today, areas of great wealth would have lower risk of suicide. When in reality, they have the highest risk. Intensified job anxiety, higher expectations, and more pressure for individual achievement (Kendall 2018, p. 6). People would normally think the poor and uneducated would have the greater risk of suicide.

Some trends that are increasing youth suicide in the United States are income, gender disparities, age group differences and risk factors. In the United States, females are more likely to commit suicide than males. Some risk factors may include not having access to healthcare, to help prevent the chance of an individual committing suicide. Trends that lead to suicide in the U.S. Military may include unequal social support or companionship. Tangible support, which an individual has someone who is willing to help them with money or assistance. Appraisal support, which requires having someone to listen and hear problems and provide useful information on how to solve them. Esteem support deals with having people show concern for an individuals well-being and have confidence in that individual to overcome any obstacle. Esteem support was found to be one of the most important factors in whether the Air Force personal had experienced severe suicidal ideation (Kendall 2018, p. 15).

Today, the sociological imagination allows us to bridge the connection between why an individuals problems and thoughts of suicide occur in relation to problems in society. Social issues are the main reason why individuals commit suicide. Being able to distinguish between personal troubles and greater social issues is the true heart of of thinking sociologically (Wiley 2015). I have observed many ways that suicide has been expressed to the public. Whether it has been on television or on social media. It does not help that almost everyone in the world has easy access to both of these and in return is exposed to the ways that some individuals live their daily lives. Seeing this puts an added pressure on individuals who may not live an extravagant lifestyle. Individuals posting on social media such as, Facebook and Instagram, expressing a new job and are advancing up the ladder of success. Individuals today are triggered by this because they may not possess the same skills and advantages that some individuals may have. Many times individuals feel that their failure to get hired is a personal problem due to a poor interview or lack of experience (Wiley 2015). Another problem in society today are structures. Structures are “common and persistent roles and relationships that shape human interaction (Wiley 2015). Relationships could be husband and wife, boyfriend and girlfriend, employee and employer. Problems between structures can lead to suicide. Whether it may be an argument between a husband and a wife or the loss of a job to an employer. Both of these are reasons in a society that lead to suicide. Individual choices of a person reflect how they see themselves. Society looks at individuals who are different from the rest and considers them to be outcasts. Individuals involved in teen pregnancy, drugs, or even who smoke are open targets for society to single out and look at differently. Social media has a big influence on depicting this and how these individuals are shown. Television shows picturing women who became pregnant at a young age and now have trouble supporting there children because they’re trying to go to school and work at the same time. All of this shows what can happen after making poor choices. Society today looks down on individuals who make the poor choices. As a result, that individual becomes ignored and feels like they are not accepted anymore in society.

Overall, C. Wright Mill’s Social Imagination has helped us to paint a bigger picture and really understand how society has an influence on the lives of its people. If we don’t make a change, suicide as a resort to end individual problems will continue to rise. It is clear that being aware of structures and institutions can drastically alter the way we view the world. They act as tools within the greater sociological imagination to help us think critically about why individuals may make choices (Wiley 2015). Being able to analyze the smallest problems with the biggest problems of an individual, will help to understand how an individual issue can become more public in society.

A physical examination reveals a height and weight appropriate 2-year-old female who appears acutely unwell.  Her skin is hot and dry. He reports that he has never had a problem like this before, but about 2 weeks ago he noticed that both his hands seemed to be really red and flaky. He denies any discomfort, stating that sometimes they feel “a little bit hot,” but otherwise they feel fine. He does not understand why they are so red. His wife told him that he might have an allergy and he should get some steroid cream. Jack has no known allergies and no significant medical history except for recurrent ear infections as a child. He denies any traumatic injury or known exposure to irritants. He is a maintenance engineer in a newspaper building and admits that he often works with abrasive solvents and chemicals. Normally he wears protective gloves, but lately they seem to be in short supply so sometimes he does not use them. He has exposed his hands to some of these cleaning fluids, but says that it never hurt and he always washed his hands when he was finished.

Scenario 3:

Martha is a 65-year-old woman who recently retired from her job as an administrative assistant at a local hospital. Her medical history is significant for hypertension, which has been controlled for years with hydrochlorothiazide. She reports that lately she is having a lot of trouble sleeping, she occasionally feels like she has a “racing heartbeat,” and she is losing her appetite. She emphasizes that she is not hungry like she used to be. The only significant change that has occurred lately in her life is that her 87-year-old mother moved into her home a few years ago. Mom had always been healthy, but she fell down a flight of stairs and broke her hip. Her recovery was a difficult one, as she has lost a lot of mobility and independence and needs to rely on her daughter for assistance with activities of daily living. Martha says it is not the retirement she dreamed about, but she is an only child and is happy to care for her mother. Mom wakes up early in the morning, likes to bathe every day, and has always eaten 5 small meals daily. Martha has to put a lot of time into caring for her mother, so it is almost a “blessing” that Martha is sleeping and eating less. She is worried about her own health though and wants to know why, at her age, she suddenly needs less sleep.

The biopsychosocial (BPS) approach to care is a way to view all aspects of a patient’s life. It encourages medical practitioners to take into account not only the physical and biological health of a patient, but all considerations like mood, personality, and socioeconomic characteristics. Since then she has had a fever off and on, anywhere between 101oF and today’s high of 103.2oF. Mom has been giving her ibuprofen, but when the fever went up to 103.2oF today, she felt that she should come in for evaluation. A physical examination reveals a height and weight appropriate 2-year-old female who appears acutely unwell.  Her skin is hot and dry. The tympanic membranes are slightly reddened on the periphery, but otherwise normal in appearance. The throat is erythematous with 4+ tonsils and diffuse exudates. Anterior cervical nodes are readily palpable and clearly tender to touch on the left side. The child indicates that her throat hurts “a lot” and it is painful to swallow. Vital signs reveal a temperature of 102.8oF, a pulse of 128 beats per minute, and a respiratory rate of 24 beats per minute.

Scenario 2:

Jack is a 27-year-old male who presents with redness and irritation of his hands. He reports that he has never had a problem like this before, but about 2 weeks ago he noticed that both his hands seemed to be really red and flaky. He denies any discomfort, stating that sometimes they feel “a little bit hot,” but otherwise they feel fine. He does not understand why they are so red. His wife told him that he might have an allergy and he should get some steroid cream. Jack has no known allergies and no significant medical history except for recurrent ear infections as a child. He denies any traumatic injury or known exposure to irritants. He is a maintenance engineer in a newspaper building and admits that he often works with abrasive solvents and chemicals. Normally he wears protective gloves, but lately they seem to be in short supply so sometimes he does not use them. He has exposed his hands to some of these cleaning fluids, but says that it never hurt and he always washed his hands when he was finished.

Scenario 3:

Martha is a 65-year-old woman who recently retired from her job as an administrative assistant at a local hospital. Her medical history is significant for hypertension, which has been controlled for years with hydrochlorothiazide. She reports that lately she is having a lot of trouble sleeping, she occasionally feels like she has a “racing heartbeat,” and she is losing her appetite. She emphasizes that she is not hungry like she used to be. The only significant change that has occurred lately in her life is that her 87-year-old mother moved into her home a few years ago. Mom had always been healthy, but she fell down a flight of stairs and broke her hip. Her recovery was a difficult one, as she has lost a lot of mobility and independence and needs to rely on her daughter for assistance with activities of daily living. Martha says it is not the retirement she dreamed about, but she is an only child and is happy to care for her mother. Mom wakes up early in the morning, likes to bathe every day, and has always eaten 5 small meals daily. Martha has to put a lot of time into caring for her mother, so it is almost a “blessing” that Martha is sleeping and eating less. She is worried about her own health though and wants to know why, at her age, she suddenly needs less sleep.

Research is what allows a nurse to practice their critical thinking skills. Research helps expand the mind by searching what the best practice might be for certain patient situations. As a nurse, the goal is to promote health and wellness for their patient. This includes using research by identifying barriers to which could give the patients a even better chance of health and wellness. “All nurses share a commitment to the advancement of nursing science by conducting research and using research evidence in practice.” (ANA,2015)  Research in nursing is an ongoing investigation that will never come to a conclusion due to the health care system being a continuous learning field. Research helps provide evidence that is proven to be applied to be practiced in clinical situations. Example: Bed baths; researching how to give a bed bath with the least amount of risk of getting an infection from bacteria. Should you use rags and a bucket of soap and water? Or should you use the vacuum sealed package of sponge pads? Which one will promote a better health and wellness outcome for the patient? Research shows the vacuum sealed bags are 99% bacteria free if used properly.

Evidence base practice is a combination of critical thinking and research and an understanding of the patient’s particular situation. This is where a nurse would take what they already know and combine it with valid research findings and use it along their scope of practice. In order to use evidence base practice, the nurse must also understand and respect the patient and their family’s beliefs. Evidence base practice can also show you outcomes of mortality and morbidity of patients with certain underlining diseases, based on the practice that was used to try and help promote health and wellness. Example: Infection control; using barrier precautions to keep patients safe from infection, such as always wiping down equipment when used on one patient to  another. Such as a thermascan when taking vitals.

Quality improvement is the assessment, practice, and analysis on what the medical field as a whole can do better to promote health and wellness. Nursing informatics is defined as the combination of nursing science, information science, and computer science (McGonigle, D., & Mastrian, K. G.,2 018). “Nursing informatics roles have taken many forms in focus and function over the last decades. Whereby the role of nursing informatics specialists will continue to evolve at an increasingly rapid rate in the and unfolding of new health care paradigms and connectivity between care providers and patients” (Nagle, L. M., Sermeus, W. & Junger, A. (2017). My focus is on how my current and previous experiences in different organizations collect blood samples/specimens from patients. Organizations should utilize existing data such as the patient’s hand band and employee identification card or numbers more efficiently. When drawing a patient’s blood sample, it should require a scan by the staff drawing the blood specimen. All employee already has an identification number, and patients have a medical identification number that is already in use for other purposes, such as when administering medications.

After the employees scan their badge or input, their identification first, then they will proceed to scanning the patient identification band before collecting the blood sample/ specimens.  scanning patient’s wrist band and employee badge will assist in determining the accurate time the blood sample was collected, the time it was received in the lab, and when the results are recorded into patients records or nurses is informed of the outcome. In some cases, I personally place multiple calls to the laboratory to get lab results for my patient. The laboratory stating the specimen is processing. When the specimen results are finally ready, the time that the sample was received initially is the time the laboratory documents the specimen results. Furthermore, a nurse leader should collect data from patients scheduled lab orders from the time the order is placed, specimen collections time, time received in laboratory and the time the results is available to other clinicians. By doing this, it will eliminate the delay, discrepancies of specimens among nurses, laboratory staffs, and provide timely efficient treatments from physicians.

Controlling of Influenza infections among elderly long-term resident of long-term care facilities (LTCFs) in New York using oseltamivir antiviral prophylaxis (Ishikane et al., 2016). It is important to control the rates of infection of influenza among the elderly patients in LTCFs so that the effects of the outbreak of the disease and the cost of treating and controlling it are lowered. This is a very important aspect in the nursing fraternity, which aims at ensuring management of all health conditions below adverse threshold level and improving the wellbeing of the elderly patients in LTCFs.

PICOT Question

In the patients in LTCFs, how does the use of antiviral prophylaxis approach control the infection and outbreak of influenza in the older patients in various LTCFs within the first 3 years of the reception of influenza outbreak report? In this question, P stands for the elderly patients that are long-term care residents of the LTCFs due to various neurological and neurocognitive medical conditions. The people who shall be recruited in the controlling of influenza infections using antiviral prophylaxis approach. The ellipsis I represent the oseltamivir prophylaxis intervention or treatment that shall be administered on the long-term care elderly residents in the LTCFs in New York. The acronym C refers to the comparison aspects, which identifies the control or reference group upon which the results of the administered intervention will be compared to. In this case, the elderly residents subjected to standard and drop precautions approaches will be the control group. The O on the other hand denotes that outcome which shall be measured in order to achieve the objective of examining the effectiveness of antiviral prophylaxis intervention on the subjects under study. Lastly, T is the Time or duration during which the intervention is effective. In this study, antiviral prophylaxis shall be administered for a period of three years. Evidence-Based Solution

The use of antiviral prophylaxis approach has been reported to decrease the occurrence of influenza infections among long-term care residents in LTCFs. The use of oseltamivir neuraminidase inhibitor shortens the clinical disease, lowers viral load and inhibits nasopharyngeal virus shedding and transmission among members of a household (Ishikane et al., 2016). In order to improve the situation for future nursing practices, efficient assessment and monitoring of etiological and epidemiological factors of influenza will be important in planning of the oseltamivir prophylaxis program. It is vital for the prophylaxis intervention to be administered during the onset of the infections months and 1 day after the report of influenza outbreak (Miksić et al., 2015).

Nursing Intervention

In order to control the outbreaks and spread of influenza disease, timely vaccination with oseltamivir is recommended at the onset of every influenza outbreak seasons. Long term care elderly residents with neurological and neurocognitive medical conditions will be vaccinated with oseltamivir at the onset of winter and summer seasons. Studies show that influenza infections are most likely to occur in December, rising to January and early February. Studies reveal that antiviral prophylaxis increases protection for about three months, which lowers in summer. This implies that summer experiences high outbreaks between June and August. Accurate prediction and administering of oseltamivir prophylaxis may improve the management and control of influenza infections in the peak seasons. Compared to use of standard and drop precautions measures, antiviral prophylaxis lowers the nasopharyngeal viral shedding, transmission and load among patients in constricted settings like LTCFs. The intervention offers 25% lower influenza outbreaks in peak seasons compared to standards and drop precaution measures. Comment by Ellen Pavela: Missing citation for this section.

Patient Care

Administering antiviral prophylaxis in elderly long-term care residents of LTCFs will reduce their exposure to and resistance to influenza virus. This will reduce the influenza outbreak incidences and thus reduces adverse effects and mortality rates among elderly long-term care residents in LCTFs. Influenza is reported to cause severe illness in over three million people and 250, 000 deaths annually in the world (Andrew et al., 2017). The highest percentage of the affected are people aged 65 years and above. Reducing influenza outbreaks will lower the medical burden on the patients and existing healthcare system, will reduce mortality, morbidity rates and lower hospitalization cases (Merritt et al., 2016).

Health Care Agency

The outcome of the study will be presented to the health service staff in the LTCF facilities in New York. The LTCF facilities in New York have short-term residents and long-term residents of the facility suffering from chronic diseases like cardiovascular, diabetes mellitus, dementia, neurological and neurocognitive diseases among others. Other occupants of the facilities include medical staff that comprise of physicians, various categories of health workers, nurses and practitioners, visitors and family members (Najafi et al., 2017). These occupants should be informed on the importance of controlling influenza. Other important agencies that need to be informed on the importance of controlling influenza include New York Communicable Disease or Management organization, the Ney York CDC and LTCF management, Centers for Medicare and Medicaid Services (CMS) among other funders and healthcare researchers (Ishikane et al., 2016).

Nursing Practice

Improving on the strategies of managing and controlling influenza infection in New York LTCFs is important in controlling influenza outbreaks in the state (Ishikane et al., 2016). This impact on the daily activities of nursing fraternity in that their health wellbeing will be improved and shall be protected from influenza infection. This will improve their health status and thus increase their productivity (Andrew et al., 2017). Daly activities will include assessing and monitoring influenza outbreak aspects, management and control measure implementation, daily assessment of the patients’ wellbeing, pain and other health condition adverse impacts and quarantine enforcements in the event of outbreak. The nursing fraternity will also be involved in information acquisition and sharing which is reported to increase the effectiveness of controlling risks of Influenza outbreaks in LTCFs by one-third. Information sharing increases outbreak awareness and antiviral programming, which play a big role in timely interventions (Cheng et al., 2018).

Conclusion

Influenza infection in the LTCFs in New York is on the rise given the dramatic change in the environmental aspects and therefore means to control the disease before becoming a pandemic is necessary. Although many approaches exist, oseltamivir antiviral prophylaxis approach proves to be the most effective in controlling the infection among long-term care residents of LTCFs. The approach reduces the adverse effects of the disease on both the patients and government spending, and also alleviates mortality, morbidity and hospitalization cases of the long-term care residents of LTCFs. Controlling influenza is beneficial to the government, families and other financial sponsors of health care systems. The knowledge of the effective approaches for influenza control transforms the daily activities of the healthcare professions and nurses by increasing their monitoring and evaluation of the aspects of influenza infections and control. Introduction and Alignment

Each student will choose pathology of specific interest and prepare a PowerPoint (PPT) presentation, APA style on the subject.  This information will be presented online during the appropriate class time.  The presentation will cover background and prevalence of the disorder, related anatomy and physiology, etiology, related laboratory and diagnostic tests (differential diagnosis), treatment, possible outcome, and complications of the disease process. You should use the notes section to add more depth to the presentation. These should not be more than 25-30 slides not including references and rubric.

You must also develop two questions that your classmates will respond to. The PPT should be posted by day three of Workshop Eight. You must also respond to four of your classmates questions by the end of the workshop.
