Improving the well-being of mothers, infants, and children is an important public health goal as their well-being determines the health of the next generation. However, the epidemiology of maternal, infant, and child health is complex. For example, the infant mortality rate among First Nations, Inuit, or non-Indigenous groups in Quebec, Canada, is significantly higher than other Canadians despite a universal health insurance system (Luo et al., 2012). Socioeconomic status, educational attainment, and other social and biological factors may influence such disparity (Bryant, Worjoloh, Caughey, & Washington, 2010).

Fortunately, epidemiologists and policy makers are developing and implementing preventive and intervention strategies. An example of this is represented in Healthy People 2020. These strategies are aimed at addressing specific outcomes such as reducing maternal mortality during pregnancy or increasing vaccination rates among infants and children. Epidemiologists are also developing novel methods to assess outcomes of the interventions.

For this  consider the current issues of maternal, infant, and child health. Then select a maternal, infant, or child health issue that impacts your community. Consider potential solutions to this issue and how you might assess success.

With these thoughts in mind:

This defense in depth discussion scenario is an intentional cybersecurity attack in Moline, IL on the water utility’s SCADA system. It occurs during the fall after a very dry summer. The water utility’s Information Technology (IT) person did not receive an expected pay raise and decides to reprogram the SCADA system to shut off the high-lift pumps. The operator’s familiarity with the SCADA system allows him to reprogram the alarms that typically notify operators of a high-lift pump failure. In addition, he prevents access to the SCADA system by others. A wildfire breaks out on the outskirts of the city. Description of public health issue

The public health issue chosen is Type II diabetes as it relates to minority populations. The variable that is being researched is whether or not compliance with medications differs between minority and non-minority racial/ethnic populations. Diabetes remains the 7th leading cause of death within the United States alone with greater than 30 million people diagnosed with Diabetes, and over 84 million diagnosed with prediabetes (CDC, 2019). Diabetes has been linked to cardiovascular disease, neuropathy, chronic kidney disease, eye damage, peripheral vascular disease affecting extremities, skin conditions, opportunistic infection, and mental illness such as Alzheimer’s or dementia (Mayo Clinic, 2018).

It has been found that 15.1% of American Indians, 12.7% of Hispanics, 12.1% of Black non-Hispanics, were diagnosed with diabetes between the years of 2013-2015 compared to only 8% of Asians and 7.4% of White non-Hispanics (CDC, 2019). For racial and ethnic minorities, it has been found that there is worse control of diabetes, higher prevalence rates, and increased complication rates (Peek, 2007). Understanding what specific risk factors these patients may have, as well as how to reduce the health disparity between races is important in improving health outcomes across all populations.

The Pros of having a single payer healthcare plan would be considered universal coverage for people that are eligible for health coverage “the coverage would also be continuous people would not have to lose coverage or need to change coverage, family status, or income” (Blumberg, Holahan 2019). The single payer coverage would guarantee that all U.S residents would have the same benefits, with no high out-of-pocket expenses. The single payer plan guarantees coverage for people with preexisting conditions, and helps people to great access to care for people that have a lower income. The health plan has equal rates for people under the plan.

The cons of the single payer health plan is that the system “some estimate that the single payer system could increase the federal health care spending by more that 50 percent” (Blumberg, Holahan 2019). The health care spending would increase government spending which leads to higher taxes, and a increase in the federal deficit. The single payer systems could lead to cuts in the healthcare organizations including cuts in Physicians pay which could add more to the shortage of Physicians.

There are people who are for and against single payer health plans. A single-payer plan is just one way that we can lead up to universal coverage. Many people do not understand what a single-payer system entails. Our healthcare system relies on the private sector. Although we have the Affordable Care Act, we have programs that are federally sponsored like Medicaid and Medicare that “provide subsidized or free healthcare to those who qualify using taxpayer dollars”. The end goal of both is to have the healthcare industry run and regulated by the government and take private health insurance companies out of the mix (Neeman, 2018). The problem is that we would have to use taxpayer money to fund the costs. “If so many other developed nations have adopted the single-payer healthcare system, then why is the US so hesitant to follow suit?” Just like everything, there are pros and cons to the matter.

Pros:

Everyone is covered. Unfortunately, healthcare is not a right. So why not switch over to Medicare for all where everyone has coverage? About 1 in 3 Americans between the ages of 27- 36 say they’ve chosen not to seek needed medical attention because they couldn’t afford it (Neeman, 2018).

Healthier population. When people don’t have insurance coverage, they are losing out on benefits, especially preventive care. It’s way easier to catch and treat something before it advances into something worse.

Better for business. Most employees have insurance through their employers. Employer- sponsored coverage is actually mandatory under the Affordable Care Act if a company has 50 or more employees (Neeman, 2018). But if we had a single-payer system, employers wouldn’t have to worry about the costly benefits.

Reduced spending per capita. The US spends the most per capita; granted we do have a larger population.

Cons:

Significant tax hikes. Just imagine how much more you would be paying in taxes. The money has to come from somewhere. For examples a “2.2% tax would be applied to individuals who earn $200,000 and to families who earn $250,000 or less each year”. This could hurt businesses causing a higher unemployment rate.

Longer wait times. It’s going to be hard to avoid these longer wait times. If everyone had access to the same coverage, there would inevitably be more people seeking medical attention that there are now, which would create longer wait times (Neeman, 2018).

Reduced government funding. This would financially drain the government. It would take money away from infrastructure and education.

Eliminating competition. “Competition drives innovation.” If there are no profits to drive the competition, doctors couldn’t compete with the best technology (which increases their profits).

It doesn’t matter whether you are for or against this idea, but you need to at least have an understanding.

Using the National Survey of Children’s Health dataset, they conducted a bivariate, multivariate, and descriptive analysis to look for associations between kids with ADHD and comorbid conditions and the strain on parents, social support, mother’s mental health, and local amenities. Results showed an increase in parental strain when caring for an ADHD child with a co-occurring condition. It also showed that lack of social support and lack of access to community amenities were predictors of increased parental strain. This study demonstrates the impact on the health of caregivers to ADHD children with comorbidities.

    Radigan, M., Lannon, P., Roohan, P., & Gesten, F. (2005). Medication patterns for attention-deficit/hyperactivity disorder and comorbid psychiatric conditions in a low-income population. Journal of Child and Adolescent Psychopharmacology, 15(1): 44-56.

    The authors examined the psychotropic medications usage of low-income kids who have been diagnosed with ADHD comparing those with and without comorbid conditions. The New York State Department of Health Medicaid Encounter Data System was used to extract information on 6,922 kids 3-19 years of age. A multivariate logistic regression was conducted to look at associations between ADHD with comorbid conditions and medication usage. Results showed the strongest predictors of medication use to be comorbid conditions and Social Security Income Medicaid eligible status. This study stimulates the question of the possibility for ADHD children with comorbidities to have treatment variations based on income status.

    Rockhill, C., Violette, H., Vander Stoep, A., Grover, S., & Myers, K. (2013). Caregivers’ distress: Youth with attention-deficit/hyperactivity disorder and comorbid disorders assessed via telemental health. Journal of Child and Adolescent Psychopharmacology, 23(6): 379-385.

    The authors examined the additional impact of comorbidities in kids with ADHD and the relationship to caregiver distress. Data was obtained from the Children’s ADHD Telemental Health Treatment Study for kids with ADHD in underserved communities. ANOVA and ANCOVA were used to look for associations between ADHD alone, ADHD plus one comorbid condition, and ADHD plus two or more comorbidities and caregiver distress. They found that parents of ADHD kids plus one or more comorbid conditions had higher levels of depression, stress, and strain compared to parents of kids with ADHD alone. This study reinforces the need to better understand the dynamics of ADHD and comorbidities and potential disparities.

· Choose one of the Ten Commandments and summarize its explanation from the “Decalogue” article.

· Note any surprises, questions, or new insights. As a class, work to provide at least one summary of each of the commandments.

I loved reading the “Decalogue” article and how it broke down the Ten Commandments. The one that I chose to elaborate on is, the fifth commandment, “Honor your father and mother, so that you may live long in the land the Lord your God is giving you” (Exodus 20:12 NIV).  The way this was explained in the Decalogue, “Honor is a translation of kabod, which means glory and interestingly enough, weighty”, which is the meaning of how much value or worth someone is. I am a Polynesian, which culturally it is strongly taught from a very young age to honor your parents and how this is shared is more than just obeying your parents when you are young but taking care of them when they are old. Also, this doesn’t just apply to your parents but to your “elders”, meaning grandparents, elderly aunts or uncles. This resonates for me because I’ve seen this my whole life. Long discusses this idea that sermons that were observed from the 1960-1980’s were taught different than from 2000-present, he states, “ a time when the large baby boomer generation is increasingly assuming care for aging parents, the sermons typically presented the fifth commandment as addressed not to youth but rather to adults charged with the responsibility to care for the elderly”. This idea is closer to what the Israelites were living since, they did have to care for the elders once they got to the “Promise land”, which leads to the blessing that follows this commandment. I love that this is the commandment that is followed with a blessing, because this is where we are able to truly come full circle.

The ten commandments are rules to live by and are provided to us by God. The purpose of the rules is to give us direction in life, help avoid committing sins and leading a wrong life. For today’s discussion, I chose the fifth commandment which says, “Honor your father and your mother, so that your days may belong in the land that the Lord your God is giving you.” (Exodus 20:12, NSRV). 

This commandant always stood out to me. When I would start acting up my mother would always reply to me by saying the fifth commandment out loud. I remember it always impacting me severely and making me get my act together. It was such a powerful sentence to me. Reason being that I would recognize how important it must have been to God to put it in the ten commandments. As I grew older I made sure to also make my daughter understand the severity of this commandment. Of course all the commandments, but when it came to respecting elders I made sure this commandment was seen and heard. As a child, I took it literally and thought it only applied to my mother and father. But as I grew older and began to think deeper into it, God is also our Father. Therefore this commandment must not only imply to my parents but also God and every other elder. I have always told my daughter that one must respect to be respected, and vice versa. Yet at times you must know when to hold back and control your words and actions, especially when it comes to those who are older than you.

Mary is a 47-year-old, single, heterosexual Caucasian female. She lives with her 52-year-old sister and 87-year-old father in the home in which she was raised. She also has a 45-year-old sister who lives 10 minutes away and a 23-year-old daughter living on her own. Mary and her family members do not maintain friendships outside the family. Mary has been unable to work for the past 3 years because she says she has felt too frightened to go too far from her home. As a result, she has been financially reliant on her family members for these last few years. Prior to this lapse in employment, she had been a school bus driver and an administrative assistant at a warehouse distribution center. Mary has no history of drug or alcohol abuse. She is well groomed and physically fit with a diagnosis of hypoactive thyroid, for which she is treated with Synthroid®. Mary was diagnosed with post-traumatic stress disorder (PTSD) and adjustment disorder, not otherwise specified (NOS) by the clinic psychiatrist.Before meeting with me, Mary saw a social worker in a private practice for 2 years. She entered treatment with that clinician because she said she was traumatized by a romantic relationship with a married African American man she had met at work. Her trauma symptoms began 6 months after she ended the yearlong romantic relationship. Mary said the romance occurred because he had “brainwashed” her, as there could be no other reason she would have slept with him. Mary believes that bad people are capable of “brainwashing” good people to perform bad deeds. Mary was raised in a home that espoused racism, and she and her family members believe that African Americans and other people of color are untrustworthy and bad. She said, “I take after my father, and he thinks black people are just evil.” Mary said she understands her feelings about race are not right.Mary considered her initial attempt at treatment unsuccessful for two reasons. First, she felt the therapist (a Caucasian woman) judged her and her family harshly for their racial beliefs and this got in the way of the two of them building a trusting working relationship. Second, she did not feel relief from her PTSD symptoms. Mary ended the relationship with that social worker 6 months ago. Mary then approached her primary care physician about her symptoms, and the doctor prescribed an antidepressant. When, after 3 months, Mary asked to have her dosage increased, the doctor suggested that she get a psychiatric evaluation and consider returning to therapy. Mary’s insurance company suggested our agency for the psychiatric evaluation and approved 10 sessions.Mary said she felt sad, frightened, and anxious most of the time. She had no appetite, slept most of the day, had no interest in dressing, and rarely left the house. When she did go out, she felt the need to be accompanied by of one of her sisters.Mary presented as angry during our initial sessions. She requested that one of her sisters attend the sessions with her, to which I agreed. My intent in agreeing to have her sister in the room was to help Mary feel safe and create a strong rapport. During the early sessions, most of what Mary said began as half sentences that she asked her sister to complete. Mary referred to her sisters as her “caretakers and minders” who “know me better than I know myself so you should talk to them.” Mary said that if she talked for herself she would get “it wrong.” The “it” and the “wrong” remained elusive in meaning when I asked her what that meant.Mary agreed, after two sessions, to meet with me alone. We used our first individual session to establish Mary’s goals for therapy. Among her goals was developing ways she could feel safer about going outside alone. Over the next eight sessions, I used cognitive behavioral therapy interventions to help Mary build coping strategies for recognizing triggers to frightening thoughts and to help her manage her anxiety symptoms. I also used psychoeducational interventions to help Mary develop routines for proper sleep hygiene, healthy eating, and regular exercise.After several sessions, Mary shared insight into her feelings about dating an African American man. Mary said that being attracted to an African American man frightened her and that there was no future for her relationship with this man because he was married. Mary believed that she had jeopardized her secure position in the family because being with an African American man challenged the family’s ideas about race and their view of themselves as separate and unique from non-family members. Once the family discovered Mary’s relationship with this man, she believed her only way back into their lives was to accept the role of a “crazy sister” in need of protection and whose judgment about people was faulty. By forming a relationship with an African American man, Mary had shown her judgment to be outside of the norm in the conventions of her family.In our final two sessions, Mary said that she no longer felt like she was the “crazy woman in the family” and she felt safe going to the grocery store alone. It was my impression that Mary may have been the identified patient in her family but exploring this idea would require family therapy.

Health professionals today are increasingly accountable for the use of protected health information (PHI). Various government and regulatory agencies promote and support privacy and security through a variety of activities. Examples include:

  • Meaningful use of electronic health records (EHR).
  • Provision of EHR incentive programs through Medicare and Medicaid.
  • Enforcement of the Health Insurance Portability and Accountability Act (HIPAA) rules.

Technological advances, such as the use of social media platforms and applications for patient progress tracking and communication, have provided more access to health information and improved communication between care providers and patients.

At the same time, advances such as these have resulted in more risk for protecting PHI. Nurses typically receive annual training on protecting patient information in their everyday practice. This training usually emphasizes privacy, security, and confidentiality best practices such as:

  • Keeping passwords secure.
  • Logging out of public computers.
  • Sharing patient information only with those directly providing care or who have been granted permission to receive this information.

Today, one of the major risks associated with privacy and confidentiality of patient identity and data relates to social media. Many nurses and other health care providers place themselves at risk when they use social media or other electronic communication systems inappropriately. For example, a Texas nurse was recently terminated for posting patient vaccination information on Facebook. In another case, a New York nurse was terminated for posting an insensitive emergency department photo on her Instagram account.

Health care providers today must develop their skills in mitigating risks to their patients and themselves related to patient information. At the same time, they need to be able distinguish between effective and ineffective uses of social media in health care.

Anti-oppressive social work means critically reflecting on your own cultural identities and how the social environment impacts these identities. Acknowledging power and privilege can be uncomfortable; however, with values of multiculturalism and social justice, social workers are committed to engaging in their own personal work and addressing social barriers clients may experience. Social workers view clients from a strengths-based perspective utilizing client strengths to support their goals, rather than pathologizing clients from the lens of the dominant culture.

For the past six weeks, you have learned about the social construction of social identities, structural inequality based on dominant and non-dominant groups, and oppressions based on sex, class, and race. While readings have continuously pointed out white privilege as the dominant group privilege, you also know that privilege is not equally distributed in groups. Intersecting identities creates unique experiences for clients. Dalia is a 14-year-old, biracial female of African and Irish American descent who resides with her parents in a middle-class suburb. She is the youngest of three children and is currently the only child remaining in the home. Dalia’s parents have been married for 25 years. Dalia’s father works in the creative arts field with a nontraditional work schedule that has him gone overnight and sleeping late into the morning. Dalia’s mother is an executive who works long hours. Dalia was diagnosed with sickle cell anemia in early childhood and was hospitalized multiple times. At present, Dalia’s health is stable with the last serious episode occurring 2 years ago.

Dalia’s parents reported that until middle school, Dalia was an easygoing, good-natured youngster who enjoyed singing and participating in activities with her peers and family. Dalia denied any problems with drugs or alcohol but admitted to drinking with friends. Dalia described her family relationships as tense, stating, “My dad lets me do what I want” and “My mother is always trying to control me.” Dalia described her relationship with her older brother, who lives in another state, as “cool,” and her relationship with her older sister, a college sophomore, as “not cool.”

Dalia’s parents sought out counseling services for behavioral issues on the recommendation of her school. The issues included argumentative behavior with authority figures, physical altercations with peers, poor concentration in class, irritable mood, verbal combativeness when confronted, truancy, and highly sexualized behavior with male peers. At home, Dalia had become more argumentative and physically threatening. Her parents had discovered that she and her friends drank alcohol in their home. Dalia’s parents also reported that she was up most of the night and slept most of the day. They also reported that her mood was highly irritable and that she was extremely impulsive. She had no interest in getting involved with any extracurricular activities, stating that those things are “corny and boring.”

In the first meeting, Dalia and her mother both appeared agitated with each other and became argumentative when going through the intake information. Dalia quickly told me that she was not planning to talk about anything because this meeting was her parents’ idea. She stated, “I don’t have any problems, my parents do.” Soon into this first visit, Dalia blurted out that her mother was upset with her because she had just shown her a tattoo she had had done recently, purchased by using a fake ID. I acknowledged her news and asked if this was the way that she usually shared important information with her mother. Dalia shrugged and stated, “I don’t know. I figure I better her tell her now before she gets too busy.”

I asked both Dalia and her mother what their expectations were for counseling and what each would like to get from these visits. Dalia’s mother seemed surprised and stated, “This is for her. She better change her attitude and start to focus on school.” I explained that often it is helpful to have sessions both individually and with family members. I pointed out that because family issues were identified it might be productive to address them together. Dalia’s mother agreed to attend some meetings but also stated that her time was limited. I was told that Dalia’s father would not be able to join us because he was never available at that time.

Dalia and I began sessions alone, and her mother joined us for the second half. During the family sessions, we addressed the communication breakdown between Dalia and her mother and Dalia’s at-risk behaviors. Individual sessions were used to address her impulsive behavior and self-esteem issues.

In individual sessions, Dalia talked about how the family had changed since her sister left for college. She said her parents stopped being present and available once her sister went away to school. She said she spent more time on her own and her behavior was under more scrutiny. Dalia also talked about her sister, describing her as an excellent student and very popular. She said her teachers in middle school would often compare Dalia to her sister, making her feel unsuccessful in comparison. During a family portion of a session, Dalia’s mother initially disagreed with Dalia’s point of view regarding how the family had changed, stating, “She’s just trying to trick you.” I encouraged them to discuss what was different about the family dynamics now compared to when the older sister was at home. We discussed how the family had changed through the years, validating both perspectives.

In time, I was able to have Dalia’s father join us in some of the family meetings. He said he felt Dalia’s behaviors were just a stage and part of being a teenager. Dalia’s parents disagreed openly in our sessions, with each blaming the other for her behavioral issues. During these sessions, we addressed how they each may have changed as their children matured and left home and how this affected their availability to their youngest child. I helped them identify what made Dalia’s experience distinct from her siblings’ and examine what her high-risk behaviors might be in reaction to or symptomatic of in the family.

In the course of the family work, the realities of being a biracial family and raising mixed-race children were also addressed. We discussed how the parents navigated race issues during their own courtship and looked at the role of acculturation and assimilation with their children in their social environments as well as respective families of origin. Educating both parents around race and social class privilege seemed fruitful in understanding distinctions between what they and their children may have faced.

After 12 weeks it was agreed that therapy would end because Dalia would be starting high school and the family felt better equipped to address conflict. The family had made some changes with the household schedule that increased parent–child contact, and Dalia agreed to more structure in her schedule and accepted a position as a camp counselor in a local day camp for the summer. Termination addressed what was accomplished in this portion of therapy and what might be addressed in future counseling. The termination process included reviewing the strategies of conflict resolution and creating opportunities for family contact and discussion in order to reinforce those behavioral and structural changes that had led to improved communication and conflict reduction.
